• Signed W-9 Tax Form
• Carrier Packet
• Proof of Insurance ($1M Liability/$100K Cargo)
• Copy of CDL and/or driver license
• USR Registration
• Notice to Assignment
• Basic tools for simple assembly work
• Your own Truck
• Motor Carrier Authority Must be180 days or more
