Purpose:
The Outpatient Care Coordinator provides initial comprehensive needs assessment, registration of clients requesting services and completion of initial paperwork required for clients to begin outpatient treatment at Western Psychiatric Ambulatory Programs. Additional responsibilities include provision of case management services including assessment, service linkage, resource development, coordination of all services received, and advocacy.
The OCC is responsible for the administrative admission process for all new patients to WBH’s Child & Adolescent Services and connecting families/patients with PHP.IOP services to efficiently access care. This position is based out of Southside, PA and works Monday through Friday, from 8:00 AM to 4:30 PM.
Responsibilities:
- Completes and documents comprehensive assessments to assist consumer in determining needs (i.e. mental health treatment, appropriate level of case management, housing, medical, legal, drug and alcohol, social supports and financial.)
- Utilizes Recovery Principles in efforts to promote consumer self-management of illness
- Works with treatment team members to ensure that clinician caseloads are current with the engagement/ attendance policy.
- Maintain case records with accuracy and completeness, including Intake documentation and Allegheny County commitment paperwork.
- Completes referrals, progress notes and other required documentation in timely manner.
- Connects and engages potential clients by answering the main ambulatory clinic phone number and offers information and referral to all callers.
- Provide case management assessment, and service linkage, resource development, monitoring and coordinating services received.
- Provide assistance with Involuntary Commitment paperwork and processes including transportation and activating emergency resources when needed.
- Facilitates and manages hospital discharge information within the electronic medical record.
- Provide necessary reports, statistics and other requested information in a timely manner.
- Facilitates access to chosen services by completing and submitting referrals to selected services and supports and provides follow up and client activity reports as required.
- Obtains identifying data, brief psychiatric history and other information necessary to facilitate continuity of care between ambulatory clinics and referral source.
- Informs and assists consumers in making choices among agency, community and county resources.
- Assures continuity of care through coordination with community supports and members of the treatment team, i.e., the psychiatrist, Outpatient Clinician, IOP Clinician, Targeted Service Coordinators, etc.
- Monitors and maintains computerized and manual accountability and tracking of intake data.
- Utilize current DSM to develop provisional diagnosis.
- Coordinates administrative functions associated with processing new referrals to the clinic and consults with program director or clinical staff related to clinical concerns.
- Advocates for consumer and/or family choice of services and active participation in his/her recovery process through education and training.
- Maintains familiarity with requirements of contracting insurance companies and referral/service requirements and obtains appropriate insurance authorizations, as needed.
- Work with relevant tx team members and attends treatment team meetings as scheduled.
- Monitors engagement process and intervenes as needed to address engagement barriers as identified.
Qualifications:
- Bachelor’s Degree in Human Services or Business related fields.
Licensure, Certifications, and Clearances:
- Cardiopulmonary Resuscitation (CPR)
- Comprehensive Crisis Management (CCMC)
- Act 31 Child Abuse Reporting with renewal
- Act 33 with renewal
- Act 34 with renewal
- Act 73 FBI Clearance with renewal
