The Patient Care Coordinator (PCC) coordinates the plan of care for assigned patients and works with physicians, case managers, social workers, and multidisciplinary care team members to facilitate clinical pathways and achievement of desired quality, financial, and service excellence outcomes. The PCC collaborates with other members of the healthcare team to ensure safe, timely, efficient, and effective care coordination that responds to the individual needs of the patient/family. The PCC is responsible for meeting and maintaining established patient/family care and professional practice standards.
Shift: Rotating 12 hour shifts (7a-7p and 7p-7a), including rotating weekends and holidays.

Experience
Preferred- 2 - 4 years: A strong clinician with two (2) to four (4) years of clinical knowledge of the targeted patient population preferred.
Education
Required- Associates in Nursing or better
- High School/GED or better
- Bachelors in Nursing or better
