Job Summary
Responsible for providing various supportive social services to assist chronically mentally ill and dually diagnosed people with managing practical problems in daily living. Responsible for administering and delivering Mental Health / AODA and supportive services to individuals and families.
Essential Duties
Participates in evaluation of client needs and strengths and assists in designing supports and interventions under the guidance of the treatment team and Behavioral Health Supervisor. Works, with the client, under the guidance of the treatment team and Behavioral Health Supervisor, to develop a client centered plan with the objective being to meet the client's life goals.
Provides regular assistance to clients in learning and maintaining adequate activities of daily living skills.
Provides support to clients and families in crisis or stress situations.
Participates in the development of client service plans and required updates under the guidance of the Behavioral Health Supervisor, or working within the context of the treatment team.
Provides case management under the supervision of the Behavioral Health Supervisor or their/designee (typically Clinical Social Workers).
Makes regular visits to clients who have difficulty with self-management of medication and reports misuse, abuse or other problems to appropriate staff.
Makes home visits/telephone contacts to clients who have difficulty keeping scheduled appointments with doctors or therapists and assists them in maintaining regularly scheduled appointments.
Serves as a link to community resources and support systems while ensuring that the care is safe, effective, client centered, timely, and equitable.
Provides assistance through individual and group contacts to improve clients' problem solving abilities.
Provides assistance to clients in finding and maintaining housing arrangements.
Works within a multi-disciplinary team, actively communicating on client needs and problem solving solutions cooperatively.
Screens and engages people who appear to have acute Mental Health and/or co-occurring issues at community sites, including agencies serving homeless persons and assists them in obtaining needed services
Assists with money management services to clients under the guidance/direction of the payee/guardian.
Provides services to improve the clients' vocational skills, involvement in, and integration into community activities.
Reports any changes in clients' condition to appropriate staff.
Acts as liaison to various contracts, community agencies, and consumer groups under the supervision of the Behavioral Health Supervisor.
Maintains appropriate and accurate client documentation.
Attends Community Treatment Program staff meetings, in-services and outside training sessions.

Minimum Qualifications Required
Education and Experience:
High School Diploma, plus three years direct care experience of adults with chronic and persistent mental illnesses; or three years of case management experience of severely mentally ill people in the community; or any equivalent combination of education, training and experience which provides the necessary knowledge, skills and abilities.
Licenses and Certifications:
Valid Wisconsin Driver's License.
Knowledge, Skills & Abilities
Knowledge of community and Mental Health and co-occurring resources.
Knowledge of and ability to utilize a computer and the required software.
Demonstrated skills in problem solving.
Good personal ADL skills.
Ability to accurately report consumer's need for treatment to the Behavioral Health Supervisor/clinical social worker, specifically with persons with chronic mental illness and/or Co-occurring issues.
Ability to remain calm in stressful situations.
Ability to relate sensitively and effectively with mentally ill/co-occurring persons and to be empathic to client needs.
Ability to problem solve, educate, and advocate for and with clients.
Ability to make accurate independent decisions for the well being or safety of clients.
Ability to work autonomously; seek advice and assistance as needed from Behavioral Health Supervisor/clinical social workers.
Ability to plan, organize and supervise social activities in the community.
Ability to initiate contact with others.
Ability to maintain strict confidentiality guidelines regarding all client conversations, records, and referrals in adherence to ethical, legal and regulatory standards and guidelines.
Ability to assist in client budgeting.
Ability to follow oral and written instructions.
Ability to communicate effectively both orally and in writing.
Ability to establish and maintain effective working relationships with staff and the public.
Ability to work the required hours of the position.
Must possess cultural awareness and sensitivity.
PHYSICAL DEMANDS:
Lift up to 50 pounds with assistance and/or carrying of objects weighing up to 25 pounds.
Extended periods of standing and walking, intermittent sitting; frequent driving.
Using hands/feet for repetitive single grasping, fine manipulation, pushing and pulling, and operating controls.
Frequent bending, twisting, squatting, climbing, reaching and grappling.
Distinguishing sounds at various frequencies and volumes.
Distinguishing people or objects at varied distances under a variety of light conditions.
Must be free from communicable disease.
