Responsibilities
- Conducts initial and annual assessments of members' medical, behavioral, and social needs.
- Educates members on preventive health guidelines and programs.
- Coordinates and organizes information from members' assessment to develop a care plan that addresses their needs and assures implemented interventions are discussed with members and their providers/supports.
- Conducts prenatal and postpartum assessments on pregnant members.
- Documents member information and contacts made regarding member's care and services provided.
- Provides information to members and providers regarding benefits, service providers and access protocols and educates members on appropriate use of medical services.
Qualifications
- Bachelor's degree or Associate's degree in social service, rehabilitation, psychology, or related field.
- 3+ years of experience in healthcare or social work.
- Experience working with Electronic Medical Records is preferred.
- Healthcare or social work experience is preferred.
- Experience with Motivational Interviewing and Trauma informed care is preferred.
- Knowledge of Medicare and Medicaid benefits.
- Proficiency in speaking Spanish, Hmong, or another foreign language is strongly preferred.
- Valid Driver's license, verifiable good driving record, and reliable transportation, with the ability to travel, as necessary, to attend off-site meetings on a regular basis.
This position can work 100% remotely or from our office in Madison, WI in a hybrid or full-time on-site arrangement, if preferred. Hardware and equipment will be provided by the company, but candidates must have access to high-speed, non-satellite Internet to successfully work from home.
