John Scott House Rehabilitation and Nursing Center is currently seeking a MDS Coordinator (16 hours/week) to join our team! Our compassionate and experienced staff will welcomes you to join our team and work with them to provide top of the line care to our residents. John Scott House is a beautiful facility located in a Braintree neighborhood that is near public transportation. COME JOIN OUR TEAM!

Requirements for MDS Coordinator:

• Must possess valid unencumbered, LPN or RN State License and current CPR certification
• Nursing degree from an accredited college or university
• At least two (2) years of MDS experience with a Skilled Nursing Facility is required

Job Duties for MDS Coordinator:

• Accurate completion of RAI for all Medicare patients in the facility
• Provides completion of the nursing section of all Medicare MDSs and RAPs
• Complete OBRA MDSs as PPS caseload allows
