Aspire Senior Living is seeking MDS Coordinator, experience preferred but willing to train a dedicated and enthusiastic nurse willing to grow their skill set in this rewarding career.
Job Description/Essential Functions:
- Coordination of facility's RAI assessments in accordance with state and federal regulations
- Collaborating with health plans, physicians and IDT (Interdisciplinary team) for assurance that resident meets eligibility and coverage criteria
- Auditing Medical records for the presence of supportive documentation for items coded on MDS
- Care plan completion
- Provide ongoing education to department managers, physicians and nursing staff as needed
- Accurate and timely completion of MDS assessments
- Attend weekly meetings to promote continuity of care and case management

Qualifications:
- Active Missouri LPN or RN license in good standing (RN preferred)
- Intermediate computer knowledge including Microsoft Office, Outlook, PowerPoint, Teams or ability to learn
- Excellent written, verbal, and interpersonal skills
- 1 year experience preferred
- Familiarity with MatrixCare is a plus
