Job description

The Dane County Human Services Department is currently hiring for an LTE Case Manager I in BHRC (Behavioral Health Resource Center). Hours of operation are 7:45am - 4:30pm, M-F (actual work hours and days are TBD) and hybrid work environment. This position is located at 818 W Badger Rd, Madison.
Limited Term Employees (LTEs) are limited to 1,200 hours in a payroll year and are not guaranteed any hours per week.

DEFINITION

Case Managers work closely with children, adults, families, guardians, service providers and natural supports. Under the direction of the assigned Unit Supervisor, this position works with individuals in the community to assess their psychosocial needs, determine eligibility for specific services, and connect them with identified programs.


Examples of Duties
Behavioral Health Resource Center (BHRC): Provide accurate and informed referral information to Dane County residents seeking Behavioral Health treatment. Work closely with community partners to identify individuals who would benefit from assistance in accessing services. Advocate for those who experience barriers to accessing Behavioral Health treatment. Communicate with providers, insurers, and other third party professionals to assist in service delivery throughout systems. Make referrals for crisis services as necessary. Provide active and back up phone consultation and drop-in support for BHRC consumers. Manage active caseload in collaboration with other team members. Document all calls, contacts, referrals and activities in database or record keeping system. Provide adequate follow-up services after making a referral to verify referral completion or to problem-solve unforeseen barriers to establishing and remaining engaged in meaningful relationships with Behavioral Health providers. Consult with other Dane County Department of Human Services (DCDHS) staff (child protection, developmental disabilities, long term care, etc.) regarding access to service and service prioritization and authorizations. Assist with development and maintenance of BHRC website, Behavioral Health resource database, and other targeted resource materials. Perform other duties as assigned.

Case Manager I

Requires a Bachelor's Degree in a human services field (social work, psychology, sociology, nursing, counseling, etc.) and the equivalent of one (1) year of full-time experience working in a treatment setting with persons with mental illness and/or substance use disorders. Experience is defined as working directly with individuals identified as part of the target population in a setting where the primary function is assessment, treatment, intervention, or observation of mental health and/or substance use conditions.

Case Manager II

All employees hired with a Master's degree and six (6) months of relevant experience or a Bachelor's degree and two and a half (2.5) years of relevant experience will be hired as a Case Manager II, range 20. Those who do not meet these criteria upon hire will be hired as Case Manager I and will advance to Case Manager II after obtaining one of these criteria. Applicants must also have the equivalent of one (1) year of full-time experience working in a treatment setting with persons with mental illness and/or substance use disorders. Experience is defined as working directly with individuals identified as part of the target population in a setting where the primary function is assessment, treatment, intervention, or observation of mental health and/or substance use conditions.


Bilingual Positions Requirements:

For designated bilingual positions, candidates must pass a bilingual test (e.g., Spanish - English, Hmong - English, etc.)

Certifications/Licenses:

Candidates may be required to pass State of Wisconsin Mental Health Functional Screen test within 90 days of employment and maintain eligibility to complete the screen. Requires a valid driver's license, access to reliable personal transportation and a good driving record.

Special Requirements:

BHRC positions may be required to occasionally work on evenings or weekends.


Background Check Statement: Some positions may require a criminal background check which can include fingerprinting due to the nature of the job's responsibilities. Wisconsin's Fair Employment Law, s. 111.31 - 111.395, Wis. Stats., prohibits discrimination because of an arrest or conviction record. Physical and Environmental Work Requirements:

Office environment. Ability to see and hear within normal ranges with or without adaptive devices. Ability to use finger, wrist, hand and arm motions. Ability to sit and stand.


Basic understanding of human development and behavior. Knowledge of current human service philosophies and approaches such as self- determination, person-centered planning, community inclusion, trauma informed care and asset based community development. Knowledge of, and experience applying case management methods, techniques, policies, procedures and treatment techniques. Knowledge of investigation procedures and applying motivational interviewing techniques. Knowledge of computer based applications including word, excel and other related software. Ability to use a computer to generate reports, complete forms, and document case activity. Ability to prepare and maintain complex records and reports. Ability to develop positive, long-term relationships with people, understand their concerns and act to address them in a timely, respectful and effective manner. Ability to quickly acquire and apply knowledge of laws, regulations, policies and procedures relating to the target population. Knowledge of and ability to access community resources. Ability to prioritize, calmly manage and resolve urgent or crisis situations. Capacity to organize large amounts of information and manage complex situations to achieve the most effective individual and programmatic outcomes. Ability to be decisive, consistent and diplomatic. Excellent personal, written and verbal communication skills. Ability to follow oral and written instructions, autonomously meet deadlines, participate in and appropriately use supervision. Ability to participate effectively as a team-member. Ability to engage and work with people from diverse ethnic and cultural backgrounds with understanding and respect for the individuals and their differences.
