In this Licensed Practical Nurse role, you will be providing in home high quality skilled nursing care to patients in the SEDONA/ COTTONWOOD area. Under the direction of the Plan of Treatment established with a physician and with input from patient/caregiver, you will assist in ensuring a better way of living and healthy recovery. As an LPN, you would report problems/potential problems to RN/Case Manager.

Role Responsibilities:
- Participation in the nursing process by providing input to the client's plan of care
- Implementing the care plan as written
- Evaluating effectiveness of care plan
- Record a patient's medical history accurately
- Provide emotional and psychological support when needed
- Communicate with patients' family or friends to provide advice, comfort and release instructions
- Other responsibilities will be assigned
Haven Qualification Standards:
- Must be a graduate of an approved school of nursing and currently licensed in the state of the HHA
- Home Health experience required
- Proven experience as LPN or similar role
- Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
- Understanding of confidentiality obligations and nursing best practices
- Excellent communication and interpersonal skills
- Compassionate and able to handle stress
- Spanish speaking a plus
