For RN/PT/OT Rate varies depending on type of visit.

ABOUT THE POSITION:
The Physical Therapist plans and provides physical therapy services to clients in their homes in accordance with the Plan of Care and Agency policies and procedures. The Physical Therapist performs functions that require substantial specialized knowledge, judgment, and skill and must be able to make judgments accordingly.
- Performs OASIS data sets, physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluation for clients who are receiving Home Health care services under the supervision of a licensed physician.
- Develops a care plan to provide direction to other health team members involved in the plan of care. Reviews and updates per Agency policy and client need.
- Plans, teaches, supervises, and counsels patients and or caregivers about the physical therapy needs to promote improvement and/or recovery, symptom management of disease process, and community resources.
- Communicates plans and changes to the physician and to the client Case Manager and other caregivers through the care plan, progress notes, and participation in care conferences.
- Perform duties consistent with state and federal standard and adhere to the company policies and protocols.
- Attends care conferences, initiates interdisciplinary referrals and participates in care coordination as needed.
Requirements:
- PT License in the State of Arizona
- AZ Fingerprint clearance card
- Updated Credentials
- CPR / BLS
- Negative TB Test Result
- Driver's License and Auto Insurance
- Flu Shot / Signed Declination Form
- Hep B / Signed Declination Form
- Covid-19 Vaccination / Signed Religious Exemption Form
- Immunization Records (MMR, Tdap, Varicella)
- For Per Diem: with at least 6 months of experience in Home Health
Others:
- Will be driving to patient's residences
- Must be able to lift 25 lbs
- Must be in good health and be able to fulfill the physical requirement
- Must be willing to drive all-over East Valley
- With Home Health Care experience preferred
- W4 type of employment
