The Intake Specialist provides the initial support and advocacy to mentally ill or substance abuse recovery adult consumers and their families. This professional collaborates with psychiatric and allied team members in assessment, diagnosing, treatment planning and intervention. This professional also provides triage for crisis clients.

Essential Dutiesand/or Responsibilities include, but may not be limited to the following:

1. Screens intake and referral calls.

A.

Reports to work on time as scheduled and at the location assigned.

B.

Provides useful and practical information to referral calls and/or clients to appropriate services.

C.

Handles crisis and referral calls in a professional and appropriate manner.

D.

Promptly accepts and processes hospital discharge calls for new/existing consumers by gathering clinical information, determining appropriate disposition, and scheduling aftercare appointments.

E.

Refers individuals who do not meet agency intake criteria to appropriate resources.

F.

Assures that clients receive services in a timely manner.

Demonstrates a strong knowledge of community resources.

2. Completes appropriate intake procedure.

A.

Demonstrates ability to assess clients' needs appropriately.

B.

Demonstrates a good understanding of agency services. Reports to work on time as scheduled and at the location assigned.

C.

Assures appropriate treatment arrangements are in place for clients.

D.

Completes all required documentation and billing submission in the agency defined time-frame.

3. Meets with clients as necessary and provides triage for crisis clients.

A.

Demonstrates appropriate crisis intervention skills.

B.

Completes accurate and appropriate written assessments.

C.

Provides treatment and appropriate follow-up referrals for crisis clients.

D.

Progress notes accurately reflect interventions and reflect clients goals.

E.

Service plans and reviews are written in a timely manner and reflect clear goals, objectives, and interventions.

F.

Service plans demonstrate client needs and input.

G.

Maintains monthly direct service of 85 hours.

4. Assures the clients receive appropriate levels of care through providing and monitoring referrals.

A.

Assesses clients accurately to assure appropriate level of care is provided.

B.

Participates in the plan of service meetings and provides appropriate levels of care.

C.

Provides linkages between programs to facilitate transfers.

D.

Practices Person Centered Planning principles.

E.

Intake coordinator will be responsible to assist and arrange for appropriate levels of care as consumers clinical situations change.

5. The therapist participates in committees and assumes other tasks as requested.

A.

Clinician participates in organizational committees as requested.

B.

Prepares necessary reports for utilization review.

C.

When appropriate, completes other assigned responsibilities in a reasonably established timeframe, within quality expectations.

6. The clinician demonstrates professional and ethical behavior.

A.

The clinician continues to demonstrate proficiency in his or her professional practice by attending regular case conferences, in-services, and workshops, and by reading professional literature.

B.

The clinician has no substantiated recipient rights violations filed against him or her.

Qualifications - To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Educationand/or Experience - Masters degree in social work, psychology or related fields from an accredited college or university required. Ability to effectively treat consumers with mental illness using solution focus or time limited treatment modalities. Preferred 2 years experience in working with mentally ill and substance abuse recovery population.

Language Skills - Excellent communication skills, both verbal and written. Ability to establish professional working relationship with all staff. Appropriately articulates the view of the agency when networking in the community.

Credentialing, LicensureSocial Work, Counseling or Psychology license with the State of Michigan is required. Employee is responsible for obtaining the required supervision for their respective licensure.
