This is an Open Career Service position with the Department of Health in Lee County.

 

This is a Regional Perinatal Linkage Coordinator position with the HIV/AIDS program in Area 8. This position is servicing Lee, Collier, Hendry/Glades, Charlotte, Desoto, and Sarasota Counties. This position is funded through the Ryan White Prevention/Patient care funds and is physically located at DOH-Lee in Ft. Myers. This position functions under the supervision of the Area 8 HIV/AIDS Program Coordinator and requires that the individual possesses a high degree of technical knowledge of diseases, healthcare services, principles and methods of epidemiology. This incumbent must possess the ability to collect, compile, and analyze data from OB/GYN, HIV/AIDS providers, hospitals and other related prenatal and perinatal provider activities. This incumbent has access to Axiom, CARE Ware, STARS, HMS, PROVIDE, Florida Shots and any other state required computer programs database.This position is classified as a sensitive position.  Incumbent handles protected health information on a need to know basis following Department of Health, HIPAA and Information Security and privacy policy.  Employee abides by State/Federal laws governing health information.  Responsible to maintain confidentiality, security, privacy and integrity of protected health information.

 

Specific duties and responsibilities include but are not limited to:

 

Patient Care:

  • Provides family centered care coordination to infants and children diagnosed and exposed to HIV/AIDS as well as access to    care until the protocol is completed for the baby at approximately 18 months of age.
  • Maintains a case load on assigned clients and families who meet the criteria of this program and participate in clinics to assist HIV diagnosed and exposed children with scheduling and transportation to and from medical appointments and other program related activities.
  • Communicates regularly with surveillance, Linkage, ARTAS, STD and other staff to assist with linkage and re-engagement activities for area 8 and Completes all surveillance Pediatric required forms.

 

Prevention:

  • Provides presentations/education on and assures that 076 protocol is instituted prior to delivery.
  • Develops a plan of care in conjunction with other members of the unit and do home visits, hospital visits, jail visits, and school visits as needed, monitor medications, keep accurate records and educate the family on all aspects of current HIV care and prevention method.
  • Promotes prevention among women of childbearing age; coordinates access to Test & Treat and PrEP services to discordant couples and other miscellaneous related duties as assigned.

 

Linkage and Reengagement specific:

  • Identifies HIV+ pregnant women who are at highest risk and appropriate for the Perinatal Linkage Program through screening and assessment.
  • Develops a written, client-centered prevention plan based on ongoing assessments of risk and progress toward risk reduction that also includes family planning, baby spacing, or sterilization if desired.
  • Provides multiple HIV risk-reduction counseling sessions during pregnancy and after birth if needed. 
  • Coordinates active follow-up by working closely with Healthy Start, Women Infants and Children (WIC), CMS, and any other case managers serving HIV+ pregnant women.
  • Participates in discharge planning of hospitalized patients as well as an adherence follow up plan until 18 months of age.

 

Activities and documentation:

  • Attends conferences/educational programs and participate in local patient care committees and report to the HAPC and surveillance officers of Area 8 and submit, statistical, and program reports. Maintains current knowledge base of HIV infection by attending/participating in designed conference calls, seminars and other educational programs.
  • Provides referrals to providers as needed, i.e. HOPWA, counseling, home health, social and psychosocial services, physicians and specialty care, etc.
  • Keeps accurate records and makes them available for periodic reviews. Prepares records, submits documents and reports as required. Ensure information about each linkage/re-engagement is accurately documented and provided to surveillance staff for submission with the line list document, and hospitals and enrolls HIV-diagnosed pregnant women into the program.

 

Teamwork:

  • Collaborates and coordinates with HIV testing, ARTAS Coordinators, and Disease Intervention Specialist (DIS) staff at County Health Departments (CHD), public and private medical providers, HIV case managers, community-based organizations (CBO), Targeted Outreach for Pregnant Women Act (TOPWA) providers, State and County correctional staff, community health center, and other HIV testing sites to identify HIV/AIDS positive pregnant women in an effort to link them into the HIV/AIDS system of care throughout area 8.
  • Liaises with all DOH in the area, CBOs, Hospitals and all other external providers with the end goal to prevent vertical transmission, treatment adherence and ongoing education through all available means.

 

Miscellaneous Activities:

  • Attends and participates in general staff meetings and staff development, such as team/staff meetings and in-service/ continuing education.
  • Attends computer skills training classes as needed.
  • Performs related duties as necessary including public health services during a disaster.
  • Maintains timely submission of EARs and timesheets.
  • Performs other duties as assigned.

  • Knowledge of methods of compiling, organizing and analyzing data for the purpose of linking patients and evaluating

  • Skill in listening to what other people are saying and asking questions as appropriate

  • Ability to plan, organize & coordinate work assignments and communicate effectively verbally and in writing

  • Ability to frequently bend, kneel, reach, and sit or stand for long periods of time
  • Ability to lift and carry up to 50 pounds as needed

  • Ability to work independently or with minimal supervision

Other job-related requirements for this position:

  • Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency.  Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
  • Must be willing to undergo FIT testing.
  • Selected immunizations or titers and/or periodic screening for tuberculosis may be recommended for this position.
  • Incumbent must possess a valid Florida Driver License and have a motor vehicle to carry out these duties.
  • Required to maintain a current Basic Life Support CPR with AED (BLS) certification.
  • This position is authorized to transport confidential information outside of established DOH-Lee facilities and must comply with parameters defined in DOHP 50-10.
