Hospice Licensed Vocational Nurse
Job Overview:
We are seeking a skilled and compassionate Licensed Vocational Nurse (bilingual Spanish) to join our healthcare team. As a Licensed Vocational Nurse, you will play a vital role in providing quality patient care and assisting in various medical procedures. Responsibilities:
- Perform patient assessments and document findings accurately
-Provides accurate and timely documentation consistent with the plan of care and prepares clinical notes.
- Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Administer medications and treatments as prescribed by physicians
- Provide nursing care for patients, behavioral health, and primary care needs
- Monitor and record vital signs, including blood pressure, heart rate, and temperature
- Collaborate with the healthcare team to develop and implement patient care plans
- Educate patients and their families on proper self-care techniques
- Maintain accurate medical records and ensure confidentiality of patient information
-Participates in personal, professional growth and development. Also participates in quality assessment performance improvement teams and activities.
-Performs other duties as assigned by the registered nurse.

- Proficient in patient assessment techniques
