Job Title: LVN – Hospice (part time or full time)

About the Role:
Allied Hospice, a subsidiary of ApolloMed, is looking for an LVN - Hospice who is interested in providing compassionate, comfort based care to patients in the San Gabriel Valley, located 15 minutes away from downtown Los Angeles, CA. Allied Hospice is a rapidly growing hospice agency looking to serve the needs of patients dealing with terminal conditions, as well as manage the patients' physical, emotional, and spiritual needs towards end of life.
Job Requirements and Working Conditions:
· This primary location for this position is in the ApolloMed office. The office is located at: 1680 S. Garfield Ave, Alhambra, CA 91801.
· In addition to working in the office, LVNs will be expected to travel to patients' homes to provide compassionate end-of-life care.
· LVNs for Allied Hospice typically work Monday to Friday, from 9 AM to 5 PM. We will ask LVNs for additional assistance outside these hours if a significant patient need arises.
· The target pay range for this role is $64,480 - $72,800. This salary range represents national target range for this role.
What You'll Do
· Travel to patient homes to provide comfort measures, such as pain medications, nausea medications, and oxygen (travel reimbursement is provided).
· Collaborates with Director of Nursing to develop Plan of Care for patients, implement IDT plans and attend IDT meetings
· Participates in all nursing and clinical services, including direct nursing care, patient care coordination, and QA of Plan of Care and nursing notes
· Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
· Coordinates patient care with different vendors such as nursing homes and pharmacies
Qualifications:
· Active and unrestricted LVN license in California
· CPR certification
· One (1) year of clinical experience

· You have the ability to multitask and problem solve in a fast pace work environment
· You have language proficiency in Cantonese, Mandarin, and/or Spanish (but this is not required).
