Hospice Chaplain and Bereavement Coordinator
Qualifications Include:
- Ordained, commissioned, or credentialed in accordance with organized religious group practices OR has a degree in theology or from a seminary.
- Comfortable in an interfaith setting.
- Hospice experience preferred.
- Acceptance of different lifestyles, cultures, beliefs, and values.
- Must be a licensed driver with an insured automobile in good working order.
- Excellent verbal and written communication skills.

As a Hospice Chaplain and Bereavement Coordinator at Lorian Health, you will:
- Conduct spiritual assessments within 5 days of hospice program admission.
- Assist in developing and implementing interdisciplinary patient care plans related to spiritual care.
- Provide spiritual support in alignment with patient/family wishes and religious preferences.
- Collaborate closely with local clergy if desired by the patient.
- Maintain and submit clinical records, evaluating the effectiveness of spiritual counseling.
- Coordinate visits with Case Managers and participate in IDG and QAPI meetings.
- Conduct religious services and offer bereavement support to staff as needed.
- Provide consultation and education on spiritual care to the IDG.
- Assume responsibility for self-development through continuous improvement of spiritual practice.
