Position Summary: Absolute Hospice Agency is seeking a compassionate and dedicated Chaplain to provide spiritual care and support to our hospice patients and their families. As a Chaplain, you will play a vital role in addressing the spiritual and emotional needs of individuals facing end-of-life care. This position requires strong interpersonal and communication skills, a deep understanding of diverse spiritual beliefs, and the ability to provide compassionate guidance and counseling to patients and their families.
Responsibilities:
Provide spiritual and emotional support to hospice patients and their families, respecting and honoring their diverse religious and spiritual beliefs.
Conduct spiritual assessments to determine the specific needs and preferences of patients and develop individualized spiritual care plans.
Offer religious rituals, sacraments, and ceremonies based on patients' faith traditions, as requested and appropriate.
Conduct regular visits to patients and families, actively listening to their concerns, providing comfort, and addressing spiritual and emotional distress.
Collaborate with the interdisciplinary team, including physicians, nurses, social workers, and other healthcare professionals, to ensure holistic and coordinated care.
Facilitate discussions on end-of-life wishes, advance care planning, and ethical decision-making, respecting the autonomy and values of patients and families.
Provide spiritual guidance and counseling to patients and families, offering a safe and supportive space to explore existential and spiritual concerns.
Offer bereavement support and grief counseling to families and caregivers after the death of a patient, fostering a healing and transformative process.
Assist in coordinating and conducting memorial services, rituals, or remembrance events to honor the lives of patients and support the grieving process.
Document all spiritual care interventions accurately and timely in compliance with regulatory standards and agency policies.
Qualifications:
Ordination or endorsement from a recognized faith group or religious organization.
Minimum of two years of experience as a Chaplain, required in a healthcare or hospice setting.
Strong knowledge of various religious and spiritual traditions, as well as an openness and respect for diverse beliefs and cultures.
Excellent communication and active listening skills, with the ability to provide compassionate and non-judgmental support to patients and families.
Ability to work collaboratively within a multidisciplinary team, recognizing the importance of integrating spiritual care with medical and psychosocial aspects.
Understanding of ethical principles and the ability to provide guidance on ethical dilemmas related to end-of-life care.
Sensitivity to cultural, religious, and spiritual differences, with a commitment to providing inclusive and person-centered care.
Proficiency in documenting spiritual care interventions and maintaining confidentiality.
Compassionate, empathetic, and self-reflective approach to spiritual care.
