SUMMARY & ESSENTIAL DUTIES AND RESPONSIBILITIES of the Home Visitor position:

Responsible for serving families in their homes as partners and resources to enable the families to advocate for themselves. Also responsible for planning and implementing scheduled socializations. This position serves as a generalist covering all service/content areas.


- Assists, informs and supports parents as a generalist in health, dental, mental health, safety, nutrition, social services, disabilities services, parent involvement, transition services, and education content areas.

- Completes home visits as required by Head Start Performance Standards and develops individualized strategies with parental input to enable families to build knowledge and skills.

- Plans and prepares for each home visit and socialization experience.

- Shares information regarding child development, children's behavior patterns and techniques to meet children's needs with Head Start/Early Head Start team as needed and with the child's parent.

- Assists parents in functioning as the child's primary teacher.

- Seeks and accepts support and guidance from Head Start/Early Head Start team members and supervisor.

- Completes records and written assessments including, but not limited to:

- Children's files

- Health data tracking system

- Immunization surveys

- IFPA

- Nutrition Assessment

- Parent intent survey form

- Referrals

- Prepares and enters data for computer input

- Health summaries, PIR report, monthly reports and correspondence with families

- Health screenings including, but not limited to growth assessments, developmental screenings, blood pressure, urinalysis, vision and hearing.

- Assists families in organizing and selecting representatives for parent committee(s).

- Provides officer training and follow-up for parents, community and other staff.

- Keeps all information concerning families, staff and volunteers confidential.

- Attends staff meetings and in-service trainings.

- Performs as a teacher in the children's socialization classroom demonstrating developmentally appropriate practice for infants/toddlers.

- Provides a warm, inviting, health, unhurried, safe, clean and pleasant environment.

- Responsible for planning and implementing socialization for parents.

- Follows up to ensure that all identified children receive medical, dental and/or disabilities services.

- Develops resource network.

- Refers and follows up to community agencies.

- Acts as resource to parents.

- Encourages parents to participate in activities.

- Coordinates with staff to ensure parent participation in activities such as socializations, field trips, classroom, kitchen, office, workshops, meetings and visits to community agencies.

- Works with staff and parents in developing community projects.

- Assists and supports parents as they move toward self, sufficiency.

- Submits all content areas and in kind monthly reports.

- Provides health/mental health and early childhood education to children, parents, staff.

- Attends agency and community meetings, workshops and trainings and Neighborhood Advisory Councils.

- Accompanies children or staff to hospital to obtain emergency treatment when necessary.

- Checks and stocks first aid kits monthly.

- Assists in yearly program self assessment.

- Participates in Child/Family Study Team process.

- Obtains information for children with suspected disabilities.

- Provides transportation for parents using agency or privately owned vehicle.

- Willingly accepts supervision.

- Follows strict infectious control procedures.

- Maintains positive relationships with families and team members.

- Models positive work ethic for parents and staff.

Other duties as assigned.

Salary Information
15.00 - 15.34 USD Hourly


QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Associate’s Degree in Social Work, Human Services, Nursing, Child Development, Early Childhood or related field with at least one-year experience with the preschool child (ages 0-5 years)

OR

Child Development Associate (CDA) and/or Family Development Credential (FDC). For Early Head Start must have the year of experience to include children 0-3 years of age.

Training and skill required in nursing /health related areas or education in child development preferred.

Knowledge of child and family health care.

Previous experience with community work and low income families.

Computer experience required.

Head Start/Early Head Start experience desirable.

Erie County resident preferred
