- Performs initial and ongoing data collection on Home Health patients/residents under the direction of the Registered Nurse/Licensed Practical Nurse.
- Implements plan of care developed by the Registered Nurse based on patient/resident physical, psychosocial, cultural, ethnic and religious/spiritual needs.
- Provides direct patient/resident care and reports condition or status changes accurately and timely to the RN or LPN.
- Demonstrates time management and prioritization of tasks.
- Documents results of patient/resident care within established policies, procedures and scope of practice.
- Follows facility policies on medication administration and treatments under the direction of a licensed nurse.
- Promote the Benedictine Mission and Core Values of Hospitality, Stewardship, Respect and Justice by bringing the Mission and Core Values into the day-to-day activities of the company.
- Performs other duties, tasks and/or projects as assigned.
- Must be in good standing on the State Nursing Assistant registry or appropriate entity
- Must complete required training for medication administration and various treatments
- Valid Class A Driver’s License
QUALIFICATIONS PREFERRED
- Previous Home Health, hospital, or nursing home experience preferred
- CPR certified
