- CPR certification
- Prior experience in Home Health is a plus.
- Familiarity with EMR Kinnser by WellSky is advantageous.

Job Responsibilities:
- Assist with Activities of Daily Living (ADLs): Provide assistance with bathing, dressing, grooming, and toileting for Home Health patients.
- Mobility Support: Help patients with mobility by assisting them with walking, transferring, and using mobility aids.
- Vital Sign Monitoring: Take and record vital signs, including blood pressure, pulse, and temperature.
- Medication Reminders: Assist patients with medication reminders and ensure compliance with medication schedules.
- Nutrition Assistance: Help with meal preparation, feeding, and monitoring dietary restrictions as prescribed.
- Hygiene and Comfort: Maintain patient hygiene and comfort, including changing bed linens and providing oral care.
- Observation and Reporting: Observe and report changes in the patient's condition, behavior, or symptoms to the nursing team.
- Documentation: Accurately document care provided, vital signs, and any changes in the patient's condition in the EMR system (Kinnser by WellSky).
- Communication: Maintain clear and effective communication with the patient, their family, and the healthcare team.
- Safety Measures: Ensure a safe environment for the patient by following infection control and safety protocols.
- Transportation: Provide transportation to medical appointments or assist with mobility as needed.
- Emotional Support: Offer emotional support and companionship to patients, promoting their overall well-being.
- Collaboration: Collaborate with the healthcare team, including nurses and therapists, to provide comprehensive care to patients.
- Patient Education: Educate patients and their families on self-care and safety measures.
- Compassion: Show compassion, kindness, and empathy while providing care to Home Health patients, who may be dealing with pain or illness.
