Job Description
POSITION SUMMARY: Assists clients in their homes/facilities with personal care activities, exercises, ambulation activities, and medication reminders. Performs simple treatments and procedures such as changing dressings per RN instruction. Provides minor household chores, meal preparation, and runs errands. Communicates with supervisory personnel appropriately.
ESSENTIAL FUNCTIONS/DETAILED DUTIES:
- Performs care according to Home Care or hospice care plan under the direction and supervision of a RN. Documents time of service and total time of visit appropriately. Documents care given and pertinent observations according to agency policy.
- Assists patients with activities of daily living according to the aide scope or practice and in compliance with all organizational, professional and industry standards.
- Reports changes in schedule to ensure that all involved are notified. Communicates to agency team through verbal reports, participation in staff meetings, and team conferences, as requested.
- Attempts to notify clients of scheduled visit times during the week prior to the visit.
- Consults RN or Director about client need changes. Reports client's condition changes to RN.
- Reports any conflict that might affect work ability. This includes concerns or inappropriate behavior in a home including mutual clients or relatives of clients. Promotes personal safety and safe environment by overserving infection control practices, following agency guidelines, and reporting unsafe situations.
- Maintains only a working relationship with clients.
- Takes care of supplies issued for use and demonstrates safe equipment practices.
- Participates in agency meetings and in-service programs to meet compliance requirements.
- Calls Director to notify of unscheduled absence as early as possible (by 7:30 am at latest) to allow for change in schedule.
- Provides Director two weeks' notice for request for vacation.
- Other duties as assigned.
POSITION QUALIFICATIONS:
- Successful completion of a formal certification training program and/or written skills test and competency evaluation.
- N.A. or H.C.A. certification and/or experience preferred.
- Valid driver's license.
- Valid car insurance to meet minimum requirements.
