About Home Health Care Aide:Provides care to agency clients in order to facilitate independent living. Work is directed by the Home Health Program Supervisor. Knows the County's Customer-Centered Culture principles, the County's Mission, Strategic Plan and Core Values and demonstrates a commitment to customer satisfaction for all customers on a regular basis.
Essential Functions
- Assists in implementing the case plan by performing specific tasks under the direction of the Program Supervisor. These tasks will be determined at the time of assessment and be incorporated in each individual case plan by the Program Supervisor.
- Recognizes and reports any change in physical, mental, or social health -of the client to the Program Supervisor in a timely manner. Under direction of the Program Supervisor, assists with re-assessment of client needs.
- Notes the client's nutrition, orientation, safety in the home, and documents activities and observations in client's electronic record following each visit per Code of Iowa and Medicaid compliance requirements.
- Attends training programs, in-service activities and weekly staff meetings as required.
- Continues to develop and improve the skills required in the functions performed.
- Observes confidentiality requirements in all aspects of client care.
- Performs all other related duties as assigned.
- Knows the products they personally produce and the customers of each product. Listens to the "Voice of the Customer "and closes gaps between customer expectations and experience whenever possible.
Minimum Qualifications & Requirements
- High school diploma or equivalent required.
- Current Certified Nursing Assistant (CNA) Certificate documenting satisfactory completion of seventy-five (75) hour CNA course and Completion of State approved Homemaker-Home Health Aide Training (thirteen (13) hours) required after hire and before starting to work with clients. Acceptable substitutions for the CNA Certificate include: completion of training or in the process of receiving training required for LPN or RN licensure; or possession of an Associate's Degree or higher in social work, sociology, family and consumer science, education or other health or human services fields and a minimum of six (6) months experience providing care in a home care or nursing home setting.
- Ability to work independently in the client's home or other appropriate site, under the direction of the Program Supervisor
- Emotional maturity and ability to use sound judgment in dealing with clients.
- Ability to effectively communicate with clients, families, community partners, and the public.
- Good physical health.
- Ability to function independently and as part of a team of professionals.
- Ability to carry groceries from car to home, push vacuum cleaner, carry laundry baskets up and down stairs and other physical requirements to care for clients.
- Excellent written and oral communication skills.
- Ability to use County email and County provided cell phones for texting, voicemail daily communication and Electronic Visit Verification.
Valid State of Iowa Driver's License required. Personal transportation and current auto
Insurance required. Must pass criminal records check and child and dependent adult abuse checks. Must meet requirements including: a person who has a record of a criminal conviction or founded child or adult abuse report shall not be employed, unless an evaluation concludes that the crime or founded abuse does not merit prohibition of employment. Completion of State approved Homemaker-Home Health Care Aide Training (thirteen (13) hours) required after hire and before starting to work with clients. Must complete twelve (12) hours of annual in-service training to meet Iowa Code requirements. Must maintain current CPR certification after hire. Must pass a County physical examination which includes a drug test after offer of employment. Regular work attendance required. Must remain awake and aware of surroundings at all times.
