POSITION SUMMARY:
Reporting to the Care Services Director, the Maurer-Heinsen Family Home Access Program Manager will be responsible for assisting with the development, implementation, and delivery of the Home Access Program. This program provides people with ALS additional support to enhance the safety and accessibility of their home. Such programs involve virtual and in-person home safety assessments, equipment recommendations, and facilitation of resources to meet the needs of those we serve.
This is a hybrid position requiring some work to be completed in an ALS Association office and some remote.
PRIMARY DUTIES AND RESPONSIBILITIES:
- Oversee the day-to-day operation of the Maurer-Heinsen Family Home Access program including, but not limited to, processing applications, scheduling home safety assessments, referring to appropriate home safety/access partners, filing, and processing invoices.
- Perform virtual home safety assessments and document findings to individuals living with ALS/caregivers, Care Service Managers, and referral sources, ex. ALS Clinic.
- Maintain and/or build relationships with key ramp vendors; ensure contracts are up to date and deliverables are met.
- Collaborate with ALS Association staff and clinical staff to ensure client/caregiver safety is met.
- Maintain records, files, and databases, providing reports as required or requested.
- Lead internal and external meetings related to the program for ongoing program success.
- Develop and monitor key performance indicators to determine the success of the program.
- Review project evaluation/surveys and recommend any modification of the program based on findings.
- Share program highlights with program donors, ALS Association staff, and community.
- This job has no supervisory responsibilities.
- Occupational Therapist, Occupational Therapy Assistant, Physical Therapist, or Physical Therapy Assistant certifications preferred.
- Familiarity with ALS and progressive disability conditions and their impact on independence preferred.
- Knowledge of home safety, ADA guidelines, durable medical equipment, and accessibility equipment preferred.
- Ability to communicate with program recipients, caregivers, and team members directly and virtually.
- Proven ability to partner with outside vendors/entities to implement program.
- Proven experience in program development, implementation, and monitoring.
- Proven experience working with, and reporting out, quantitative and qualitative data.
- Ability to work independently; must be a self-starter.
- Ability to manage multiple projects simultaneously.
- Proficient in Zoom, Teams, and Microsoft Office products.
- Ability to deliver presentations in small and large group settings.
- Ability to travel on ALS Association business as required or requested.
Pay Transparency: The ALS Association’s pay range for this position is $49,175 - $69,392 annually.
The ALS Association has compensation ranges for various work locations throughout the United States, allowing us to compensate employees competitively and consistently in diverse geographic markets. The above-listed listed range shows prospective compensation for this role; the appropriate range will be established by what is eventually determined to be the candidate's primary work location. Various factors, including the complexity and significance of the role, job duties/requirements, relevant experience and abilities, and primary work location, determine individual compensation. Compensation ranges are evaluated and typically adjusted on a regular basis. Offers are made within the compensation range that is in effect at the time of the job offer.
