JOB SUMMARY:
It is the responsibility of the Health Information Management Coding Specialist (H.I.M.) to manage the medical records agency-wide assuring the accurate and proper maintenance of patient information and medical records in accordance with agency policies and procedures and in compliance with requirements set forth by external regulatory agencies. Accurately performs coding on all diagnoses, procedures, professional services using ICD-9-CM, ICD-10-CM, E & M, and other CPT codes. The coding function provides/patient continuity, accurate database information, and the ability to optimize reimbursement. The H.I.M. Coding Specialist
will serve as a resource to Patient Family Care Coordinators, Team Assistants and other Hospice staff.
QUALIFICATION REQUIREMENTS:

- Associate Degree in Health Information Management required or an RHIT; or other coding credentials in lieu of degree.
- Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures and governmental regulations, as well as documents such as safety rules, operating and maintenance instructions, and procedure manuals.
- Ability to write routine reports and correspondence.
- Ability to speak effectively before groups of customers, patients/families or employees.
- Knowledge of medical terminology and ability to demonstrate essential usage.
- Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
- Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Ability to operate a facsimile, copy machines, computers and other common office machines, with working knowledge of MS Word, Outlook, Excel and other software appropriate to positions.
ESSENTIAL JOB RESPONSIBILITIES:

- Maintains the agency’s medical records within standards established for healthcare.
- Consistently audits medical records to ensure compliance with state and federal licensure regulations and accreditation standards.
- Monitors physicians initial/recertification orders and face to face encounter process to ensure compliance with CMS guidelines.
- Completes physician orders software process.
- Monitors new admission packets for completeness and notifies responsible staff member of deficiency.
- Reviews medical records to ensure all entries are signed, assembled in correct chart order and contain required forms.
- Completes chart deficiency process.
- Completes daily discharge audit process.
- Completes closed chart review process.
- Files medical records forms in correct chart and order.
- Monitors transcription files for accuracy and availability for signature daily.
- Completes ICD-9-CM, ICD-10-CM, E & M, and other CPT coding for billing hospice patients, physician services and other companies as assigned.
