Job Summary
Home Assist Health Interns gain experience and skills for addressing social determinants of health through home-based interventions and earn both a Direct Care Worker and Direct Support Provider certifications. They assist with community research, administrative duties and participate in job shadowing. Interns provide support to care plans by assisting clients with resource engagements (i.e. food resources, and transportation), and developing tools for improved skills in self-management of health and wellbeing. Upon completion, interns will have a better understanding and positioning for careers in health and social services.
Job Functions
- Act as a positive representative of Home Assist Health and upholds the company's mission, vision and purpose.
- Assist with miscellaneous and administrative duties such as inbound/outbound communication, and preparation of home-care kits and welcome packets.
- Assist with care plan activities including but not limited to appointment scheduling and reminders, community resource engagement and applications.
- Review and update community resource library.
- Participate in Case Management Huddles.
- Research community resources for targeted SDOH.
- Maintain community resources library
- Performs other duties as assigned.

Education
- Currently in High School or related training program
Knowledge, Skills and Abilities
- Requires the ability to read, write and speak effectively in English. If unable, must provide interpreter for interview, physical, and training classes.
- Good communication skills and the ability to follow directions, policies and procedures.
- Additional experience or training may be required for specific services.
- Reliable, insured transportation or the ability to utilize the transit system to ensure clients scheduled hours of care are covered
- Bilingual a plus.
Required Training During Internship
- AHCCCS Direct Care Worker Training
- Current CPR and First Aid Certification
- Article 9
Background Requirements
- Must have DPS Level 1 Fingerprint Clearance (or be able to obtain one within 90 days hire).
- Must pass Criminal Background.
- Must pass APS Registry Check at new hire and annually.
- Must sign notarized Criminal History Affidavit at new hire and every 3 years.
Immunization Requirements
- Must have proof of TB Clearance.
- COVID Vaccine required for patient facing/shadowing
