Health Coach (Bilingual)

Job Title: Bilingual Health Coach  (English and Spanish Fluently) 

Job Overview

As a Bilingual Health Coach, your primary responsibility is to guide individuals, that speak Spanish and/or English, in achieving their health and wellness goals through personalized coaching, education, and motivation. You will work closely with patients to assess their current health status, develop customized wellness plans, and provide ongoing support to facilitate positive lifestyle changes. 

Key Responsibilities

Content Development:

• Using your knowledge to develop programming for areas of GI, Diabetes, etc. 
• This development will be writing content, that is evidence based to help our members improve their health. 
• Translating English content into Spanish content 

Client Assessment:

• Conduct thorough health assessments, considering physical, mental, and lifestyle factors. 
• Analyze client health history, goals, and preferences to create personalized wellness plans. 

Wellness Plan Development:

• Design comprehensive and realistic health and wellness plans that align with the clients’ objectives. 
• Provide nutritional guidance, exercise recommendations, stress management techniques, and other relevant strategies. 

Individualized Coaching Sessions: 

• Conduct one-on-one coaching sessions with clients to review progress and discuss challenges. 
• Offer guidance on goal setting, behavior change, and sustainable lifestyle modifications. 

Education and Empowerment: 

• Educate clients on the principles of health and wellness, including nutrition, exercise, stress management and more as we develop our programming. 
• Empower clients to make informed decisions regarding well-being and develop ownership over their health. 

Monitoring and Evaluating: 

• Track clients’ progress through regular assessments and measurements. 
• Adjust wellness plans as needed based on client feedback and results. 

Documentation and Reporting: 

• Using the AZOVA software coaching sessions will be documented meeting any regulatory requirements set by AZOVA and/or contracting requirements. 
• Maintain accurate and confidential member records, documenting session details, goals and progress. 
• Prepare reports and summaries for clients, highlighting achievements and areas for improvement. 

Collaboration with Other Professionals:

• Collaborate with other health professionals to offer holistic support. 
• Refer clients to specialized services when necessary. 

Stay informed on Health and Wellness Trends: 

• Stay abreast of the latest developments in health, nutrition, and wellness. 
• Continuously update knowledge and skills through professional development. 

Qualifications: 

• Bachelor's degree in a relevant field such as Health and Wellness, Nutrition, Exercise Science, or a related discipline.
• Clinical background / Functional Nutrition or Holistic Nutrition Degree
• Certification as a Health Coach or related credentials is highly desirable.
• Previous experience in Health Coaching, counseling, or a related field is preferred.
• Strong interpersonal and communication skills.
• Empathy and the ability to build a rapport with clients.
• Knowledge of behavior change theories and motivational interviewing techniques.
• Familiarity with current health and wellness trends.
• Required: Fluent in English and Spanish 

Working Conditions:

• The role is primarily a virtual coaching position but may involve in-person and coaching sessions 
• Flexible working hours, including evenings and weekends, to accommodate client schedules.
