Health Care Coach Position for Remote
Patient Monitoring Near Sun City

ViTel Health, Inc is searching for a new member on their clinical health team in Sun City, Arizona who is personable and reliable.
Must be Certified and Accredited as a Health Coach to provide telephonic coaching and patient oversight for remote patient monitoring and chronic care management.

We are a healthcare company focused on transforming healthcare from a model of intervention to prevention with a focus on holistic and humanized care. We provide monitoring of vital signs to minimize critical health events that lead to hospitalization and death. In addition, we match all of our physicians and patients with a Health Care Coach to optimize their health and longevity.

A Vitel Health Care Coach will develop close relationships with their patients to set up the devices, engage with the patient, monitor and manage notifications, and have regular communication with the patient’s treating physician. They will be focused on encouraging and motivating the patient to consistently take their vital sign readings, develop SMART goals, adhere to the plan of care, and refer to the physician as needed.

Candidates must be:
● Accredited from a recognized Health Coaching Alliance or organization
● Experienced and Skilled communicator with experience in customer service in the medical field.
● Experienced and skilled in learning and using new software applications
● Team oriented
● Experienced in chronic care management or health care coaching with a minimum of 2 years work experience.
● Technologically Savvy
● Experienced in remote work and telephonic coaching

● Residing within 40 miles of Sun City Arizona

We prefer the following interests, training, experience and background.
● Currently licensed as an LPN, Medical Assistant, Social Worker, Pharmacy Tech,

Nursing assistant, Social work tech,etc.
● Understand of or training in Remote Patient Monitoring and Chronic Care managements requirements, components, and billing
● Understanding of Social Determinants of Health
● Interest in leadership roles and desire for career growth in the future..
● Background, education, or interest in nutrition, integrated medicine, and/or lifestyle medicine
