Position: Health and Housing Specialist
Compensation: $23.00-$25.00 per hour, depending on experience
Hours: Full-time position, 40 hours per week. Applicant must be available
Monday - Friday. Hours are flexible.

Position Summary
The role of a Health and Housing Specialist at CASH is to assist clients in finding and maintaining medical benefits and Housing solutions. This entails working with clients to create plans of action, managing their medical needs, and ensuring that they are able to access suitable housing. The Health and Housing Specialist collaborates with medical providers, housing partners, government organizations, and community groups to provide support that is easily accessible, respectful, and free of any stigma.
Work Duties and Responsibilities
· Carries out all duties and responsibilities using a person-centered and trauma-informed approach. Interactions are culturally responsive and safe and follow agency protocols using best practices.
· Maintains regular communication with partners to know available spaces for emergency, transitional, and permanent housing.
· Assists clients in locating appropriate and affordable housing, and advocate for clients in unsafe or neglected properties.
· Advise and direct clients in developing and implementing an action plan to secure housing.
· Initiate and maintain communication with case managers in supporting their clients’ success in housing and accessing medical care.
· Provide transportation to clients as necessary.
· Referrals – Connect patients to relevant community resources, mental health, and behavioral health services. Facilitate eligible patients to enroll in Medi-Cal and Cal AIM
· Group Work – Facilitate or co-facilitate therapeutic or psychoeducational groups, as needed.
· Communication– Serve as a bridge between patients and the medical team, ensuring all concerns, especially those related to psychosocial aspects, are conveyed.
· Maintain accurate case notes, client data, and other records according to agency, funder, and professional standards. Assist in tracking client outcomes and service trends to promote ongoing evaluation and improvement of services.
· Collaboratively develop patient goals, monitor progress, address barriers to goal achievement, and evaluate outcomes of individualized care plans.
· Work with clients to access medical and mental health benefits.
· Attends a variety of meetings, trainings, and seminars, which may include county training sessions, Agency training programs, and networks with other service providers as a representative of CASH.
· Learns and understands the CASH’s organizational philosophies, values, policies, operations, and program services.
· Evaluate complex patient and family situations that have legal, ethical, and psychosocial components.
· Network proactively with and demonstrate a working knowledge of community resources
· Take creative problem-solving approaches to address complex situations.
· Utilize critical thinking and conflict resolution skills to balance competing needs and responsibilities effectively.
Other Required Responsibilities
· Ensures compliance with all Community Against Sexual Harm ethical standards, and safety and security protocols.
· Attends mandatory yearly training and staff meetings.
· Completes HT 40-hour training within the first 60 days.
· Completes CPR/First AID and Mental Health First Aid within the first 90 days.
· Performs other duties as assigned.
Qualifications:
· Passion and commitment to providing services and increasing awareness of commercial sexual exploitation.
· The ideal candidate will have lived experience with commercial sexual exploitation.
· Basic knowledge of Emergency, Transitional, and Permanent Housing solutions, and the ability to advocate on behalf of clients in navigating and negotiating those systems.
· Previous experience with advocacy, outreach, and community resources preferred.
· Effective time manager.
· Ability to maintain all confidentiality requirements.
· Strong written and verbal communication skills. Comfort working in a team environment with colleagues, community partners, consumers, and all levels of management.
· Ability to multi-task and prioritize projects and assignments effectively.
· Strong team player.
· Ability to work in a mobile, fast-paced environment.
· Desire to work in a small office environment where each employee assumes multiple responsibilities.
· Flexible demeanor and ability to address conflict constructively.
· Excellent interpersonal, relationship, and team-building skills.

WORK ENVIRONMENT:
This position is performed on-site at our Center for Women and at partnering agency sites, and in the general community. This is not a work-from home position. The work environment requires comfort with multiple activities and a diverse population of people.
