Field Case Manager (Rn/Lmsw/Lpc)

We’re seeking a Waiver Case Manager/Medical Management Specialist in Downers Grove/Chicago IL area.

This hybrid role offers the flexibility to do 70% of traveling to each member’s home/facility and to work from home 30% of the time. You'll be responsible for conducting medical management and health education programs for members on government health care programs, helping them navigate and access services and support through waiver programs.

Qualifications:

• Registered Nurse (RN), LMSW, or LPC; Current, valid, unrestricted license in the state of operations
• 3 years wellness or managed care experience
• Strong communication and analytical skills
• Current IL driver's license, transportation and applicable insurance
• PC proficiency

Preferred Qualifications:

• 3 years clinical experience

• Patient education experience
• Condition Management experience
• Bilingual in English and Spanish
• Transition of Care experience
• Experience in managing complex or catastrophic cases
• Certification in Case Management, Training, Project Management or nationally recognized health care certification
