Athene Nursing & Rehabiltation is a leading provider of long-term care services dedicated to ensuring the well-being and quality of life for our residents. We are currently seeking a Full Time experienced MDS Coordinator to join our team at the Town & Country, MO location. The ideal candidate will be a licensed LPN or RN with a strong background in completing assessments and developing care plans in compliance with state and federal regulations and must have recent MDS experience in the past 3 years.

Position Summary: As an MDS Coordinator, you will play a vital role in the comprehensive care of our residents by completing thorough assessments and developing effective care plans. The successful candidate will be responsible for creating care-plan reports, monitoring adherence to care plans, and ensuring proper documentation to guarantee the highest standard of care for each resident.

Responsibilities:

  • Complete Assessments: Conduct comprehensive assessments for all residents in accordance with state and federal regulatory guidelines.
  • Care Plan Development: Develop care plans within specified time frames, ensuring accuracy and compliance with regulatory standards.
  • Report Creation: Generate clear and concise care-plan reports for the facility staff, providing them with actionable information to deliver optimal care to residents.
  • Monitoring Adherence: Regularly monitor and assess the implementation of care plans, ensuring that residents receive the prescribed care and interventions.
  • Documentation: Maintain accurate and up-to-date documentation for each resident, adhering to all relevant protocols and guidelines.

Qualifications:

  • Recent MDS Experience: Minimum of 3 years of recent experience as an MDS Coordinator in a long-term care setting.
  • Licensure: Active LPN or RN license in the state of Missouri.
  • Regulatory Compliance: Thorough understanding of state and federal regulations governing long-term care facilities.
  • Communication Skills: Strong written and verbal communication skills to effectively convey care plans and collaborate with the facility staff.
  • Detail-Oriented: Ability to pay meticulous attention to detail in documentation and care plan development.
