Direct Support Professional

Responsibilities:

• Deliver services as directed and described in each individual’s service plan and CF policy, and document as required.
• Deliver services appropriately and professionally at all times.
• Participate in all meetings as assigned.
• Ensure individual files are maintained in a manner that meets regulatory requirements.
• Comply with all state and federal regulations and CF policy and procedure. Notably HIPAA and confidentiality requirements.
• Provide services as directed and described in each individual’s service plan and CF policy, and document as required.
• Notify supervisor of any concerns noted, or expressed by individuals, family members, and other team members.
• Report MUIs timely as required by state regulations and CF policy and procedure.
• Participate in the quality assurance and service delivery oversight processes as required by CF policy and procedure.
• Report all health and safety issues to supervisor as required.
• Transport individuals as required and following CF policies and procedures.
• Other duties as assigned.

Job Details:

• Full Time or Part Time
• All shifts, including weekdays and weekends
• Competitive hourly rate: $18/hr plus potential longevity add-on and site shift differentials
