Job Description
● Demonstrates quality assurance and improvement monitoring, reviewing, and interventions.
● Complies with all established state legislation, mandated program standards, and all triumph residential services, llc requirements regarding individual services and documentation of services.
● Completes, submits, and makes corrections on daily record documentation and reviews all documentation daily.
● Attends and successfully completes all scheduled training and staff meetings.
● Demonstrates the ability to engage individuals in the community setting in age and functioning-appropriate activities.
● Demonstrates knowledge of relevant community resources with a focus on health and safety.
● Supports individuals with a wide range of disabilities in maintaining and improving skills to work toward their highest level of independence.
● Demonstrates the ability to individualize support to meet the unique needs of all individuals.
● Implements and documents progress toward outcomes and the use of behavioral interventions according to information identified in the individual plan of support. Provides on-site supervision, support, and assistance to ensure the maintenance of individuals’ health and safety.
● Provides on-site supervision and training to individuals in a residential setting, to include general monitoring.
● Provides assistance and support with transportation as needed.
● Completes and submits all programmatic, financial, audit reports, and forms, according to identified deadlines.
● Assures smooth operation of the residential homes, including maintaining a clean and safe environment.
● Monitors and recommends when supplies are needed. Communicates consistently and professionally with coworkers and supervisors.
Qualifications
● High school diploma/GED, some college preferred, and previous experience working with individuals with intellectual disability or equivalent combination of training and experience.
● Must be knowledgeable about and able to provide support for individuals with dual diagnoses.
● Must be 18 years of age or older.
● Must possess a valid driver’s license and an acceptable driving record.
● Must be able to pass extensive background check and a Virginia registry check and provide a current tb test with a negative screening.
● Must demonstrate effective oral and written communication, interpersonal and public relations skills.
● Must possess a caring and positive attitude, enjoy working with a diverse population, and display flexibility to individuals’ changing needs.
● Prefer DSP certified in medication management, CPR/First Aid, and TOVA
Schedule:
- 12 hour shift
- Every other weekend off
- Overtime
- Weekend availability
- Weekends only
