MISSION FOR THIS POSITION
The Direct Support Professional accomplishes individuals' direct care by following the ISP plan and meeting their goal to live a fulfilled life and to assure that their health and safety needs are met within the home and out in the community.
**The current job opening is for Monday-Thursday with the hours of 12:00pm-5:00pm, in the Norfolk, VA area**
DUTIES AND RESPONSIBILITIES
· Provide support and living skill training to individuals. Services may be provided at their homes or in the community.
· Assist with daily living skills such as meal preparation, menu planning, grocery shopping, cooking, cleaning, laundry, and personal shopping.
· Support individuals in leading self-directed lives by collaborating with individuals and others to plan and implement supports that build upon individual strengths, choices and capabilities that lead to inclusion into the community.
· Assist in the individual’s personal care as needed (including dressing, grooming, feeding, toileting, positioning or medical/health needs).
· Report evidence of abuse, neglect and other health and safety issues immediately. Complete incident reports and give to supervisor timely for review.
· Develop and maintain an open line of communication for pertinent information about individuals with coworkers, caregivers or employer at beginning and end of shifts.
· Provide skill reinforcement activities in areas that include socialization, communication, exercise, recreation, interpersonal skills, financial matters, civil rights, and self-determination.
· Provides transportation for individuals receiving services.
· Completes daily documentation as required.
COMMUNICATION SKILLS/TECHNOLOGY SKILLS
· Able to read and effectively interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals and follow them accordingly and consistently.
· Able to compose well-written routine reports and correspondence. This includes but is not limited to: proper spelling, punctuation and grammatically correct sentence structure that express occurrences in a concise and logical manner.
· Ability to follow the organizations writing guidelines necessary to appropriately document support logs and other necessary documents.
· Communicates with individuals in a professional manner, provides corrective feedback in private, and intervenes only as much as needed for safe completion of tasks.
· Working knowledge of computers to allow employee to access computer systems and applications to complete timecards, read and respond to email, and complete required on-line training.
· Knowledge and/or ability to develop competencies involving use of supportive technology and adaptive equipment to support individuals.
SKILLS AND ABILITIES
· Effective interpersonal and communication skills are required in order to develop and maintain positive relationships with adults with disabilities and provide a positive environmental setting.
· Ability to add, subtract, multiply and divide (basic arithmetic).
· Ability to use emergency equipment and supplies, and to respond professionally in a variety of emergency situations.
· Ability and willingness to professionally support individuals and think critically in a crisis situation. Such situations could include psychiatric, behavioral and medical emergencies.
QUALIFICATIONS
· Valid driver’s license
· Must have reliable transportation and must use personal vehicle to provide transportation to clients in their care
· Must provide auto insurance
· High school diploma or equivalent
· Must pass mandatory drug screen
· Pass a criminal history and personal background checks
PHYSICAL DEMANDS/WORK ENVIRONMENT
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
· May work 20 hours.
· The employee frequently is required to stand, walk and make twisting /bending motions.
· The employee is occasionally required to sit; climb or balance; and stoop, kneel, crouch, or crawl.
· The employee must regularly lift and/or move up to 20 pounds, frequently lift and/or move up to 40 pounds, and occasionally lift/move up to 50 pounds individually.
· Lifting of individuals in supports should always be done with caution and with consideration for the amount of employees lifting, the use of assistive devices and the weight of the individual.
