Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.
This position is based in the Day Habilitation Community Support program within CalAIM. This individual will be responsible for assessing member qualification and appropriateness for services, building and
implementing life-skills curricula, scheduling, tracking and arranging sessions at various locations,
documenting in IF’s EHR as well as MCP portals, submitting inputs for service authorization in MCP portals, and fulfilling any other unenumerated Day Habilitation need that may arise.
Pay range for this position is $23.00 - $25.00 per hour.
Responsibilities:
Research and Development
- Researching, collaborating on, and assembling life-skills curricula across all of the topics of focus designated by contractors / program administrators.
Tracking, Assessing, and Documenting
- Assessing viability and appropriateness of prospective life-skills attendees
- Tracking attendees, and, subsequently, inputting the requisite attendance notations in the provider portal as well as IF EHR
- Implement workflow for assessing / tracking new prospective attendees as they admit into other CalAIM adjacent programs such as recuperative care / short-term post-hospitalization
Scheduling and Preparation
- Scheduling upcoming life-skills modules, which includes making necessary site accommodations, connecting with site managers / facility staff to designate space, making necessary arrangements with third-party entities participating in or hosting modules, dressing / furnishing the designated space to a point of suitability, etc.
Life-Skills Implementation and Client-Care
- Some modules will not be able to be hosted by third-party entities, and thus will need an instructor assigned to them. The Peer Mentor will be responsible for navigating this assignment.
Minimum Qualifications:
Required:
- High School Diploma or equivalent.
- 2 years relevant experience.
- Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.
- Valid CA Driver’s License and eligibility for company vehicle insurance.
Preferred:
- Bilingual in English and Spanish.
- Proficiency in Google Suite (Mail, Docs, Sheets, Calendar).
