responsible for providing services to Cuban and Haitian humanitarian parolees as outlined in the
CHEA program in accordance with New American Pathways policies and procedures. The case
manager serves as the point of contact for Cuban and Haitian clients, providing appropriate
management and support services. The Case Manager will provide support to Cuban/Haitian parolee
clients in Atlanta for a period of the 90-day service period. In addition, the incumbent will work
closely with sponsors or close families to ensure services are delivered in a timely
manner. The position will involve frequent in-person work in the field. The Case Manager
should be available to work both remotely and in the New American Pathways office.
KEY RESPONSIBILITIES
- Provide direct support for Cuban/Haitian parolee clients applying for Social Security
- assistance, and application for state social services (food stamps, Medicaid and Refugee
- Cash Assistance) EAD application, health screening, etc.
- Work closely with Sponsors, close family members or Cuban/Haitian community
- organizations who provide core services to Cuban/Haitian families – including housing, ESL/
- school registration
- Conduct cultural orientation both in person and over the Zoom on various essential
- integration topics useful for the Cuban/Haitian humanitarian parolee’s clients
- Provide detailed information for clients about Cuban/Haitian parole status, compliance with
- conditions of their parole, and referral to immigration services
- Accompany clients to appointments as needed for services (e.g. social security, DFCS
- benefit enrollment, ESL/school registration/WIC.)
- Refer clients to available programs and resources for ongoing support, both within NewAP
- and to community partners, as needed for immigration assistance, medical care,
- employment services, food assistance, clothing, household items, and other community
- services for which they may be eligible
- Coordinate medical and other appointments, including arranging/providing interpretation and
- transportation
- Administer direct client assistance funds for each eligible Cuban-Haitian client in accordance
- with NewAP procedures, including maintenance of financial ledger for each case
- Liaise with community sponsors, community partners, and other community members for
- coordination of service provision for individual families
- Maintain client case files, case note log, and data needed for all reports, ensure that they are
- up to date and accurate
- Provide intensive case management support, client accompaniment, advocacy, and
- communication with community service providers on behalf of the clients.
- Maintain regular contact with eligible Cuban-Haitian families throughout the 90-day program
- period
- Fulfill reporting requirements, including 90-day client reports, and monthly activity reports
- Assist with ongoing services provision, including benefits renewals, social adjustment
- services, and workshops
- Provide language interpretation and translation across departments as needed
- Collaborate with other Case Managers, employment team, AmeriCorps service members,
- volunteers, and other staff for service provision
- Attend meetings and training/planning groups as assigned
- Work occasional evenings and weekends to respond to the emergency situations
- Other duties as assigned
- Knowledge of Spanish or Creole language
- Minimum of two years’ experience in case management, social work, or human services field
- A personal passion to assist refugees and immigrants with their efforts to gain and maintain
- self-sufficiency
- Ability to handle the volume of detail involving confidential information and requiring
- execution of assignments without close supervision
- Ability to build rapport with diverse communities.
- Flexibility and ability to problem-solving
- Ability to work in a team to achieve goals.
- Ability to effectively work remotely and utilize technology including Zoom and MS Teams
- Availability for the evening, weekend, and early-morning appointments as needed
- Driver’s license, good driving record, and access to a reliable personal vehicle required. This
- position involves frequent use of agency and personal vehicles to transport clients/fieldwork.
This is a full-time hourly position. The pay range for this position is $18.50-$21.15 per hour ($38,500-$44,000 annual) Employer contributes to medical, dental and vision insurance. Employer pays life and disability insurance. Fifteen (15) paid holidays and a generous paid time off policy. Employee has the option to contribute to 401(k).
