Position Summary
This position plays a crucial role in ensuring that our clients receive the care and support they need in a timely and efficient manner. The Medicaid Case Manager will directly contribute to our mission of delivering quality care by navigating the complexities of Medicaid on behalf of our clients.
Essential Duties
- Oversee the Medicaid application and renewal processes for Aged, Blind, and Disable (ABD) clients.
- Collaborate with community providers and partners to ensure comprehensive support for clients.
- Utilize the Gateway system for case tracking, documentation, and reporting.
- Provide expert guidance and support to clients and their families throughout the Medicaid application process.
- Follows policies and procedures to contribute to the efficiency of the office.
Required Knowledge, Skills, Abilities and Competences:
- Must have hands-on experience with Aged, Blind, and Disabled (ABD) Medicaid processing, demonstrating a deep understanding of the complexities and requirements involved in handling such cases.
- Must have a solid background in navigating the Medicaid application process efficiently, ensuring timely and accurate submission of applications.
- Experience is crucial for fostering strong relationships and ensuring collaborative efforts in case management.
Qualifications
- High School Diploma or GED or related experience in lieu.
- Experience working with community providers or partners
- Experience with the Gateway system is highly desirable
- Proficient in using Gateway for case management purposes.
PREFERRED QUALIFICATION:
- Associate degree preferred
- Minimum of two-year related experience preferred, preferably in healthcare.
