The Highland Rivers Behavioral Health Developmental Disabilities program provides activities, services, learning opportunities and supports to help individuals with intellectual and/or developmental disabilities live independently in their communities. The program provides year-round, quality-driven social, recreational, vocational and educational programs for individuals, and provides support to their families.
Highland Rivers Health is currently seeking a Case Manager in Cobb County.
Job Responsibilities include:
- Monthly contact with individuals
- Monthly documentation (Summaries, HRST updates, CASE MANAGER notes, DDP ISP review notes Individual Service Plan reviews, development, and implementation (ensure instructors have new goals after ISP)
- Training for staff as indicated in individual's service plans/HRST.
- Liaison with community, support coordination, administration, families/caregivers, and contractors, regional office, intake and evaluation, transportation, DFCS, social security Intimate knowledge of DD services and funding sources (NOW/COMP waivers, state funded) service rates.
- Manage, maintain and audit of electronic health records, including ECR page Document review (PA, DMA6 & DMA7, ISP, HRST, SIS)
- Complete trip orders for transportation/assigning staff for weekly bus routes, as needed assist with admissions/transition (admission paperwork,)
- Completes discharges and paperwork.
- Assume responsibilities for Program Manager during their absence.
- Crisis intervention, monitor and maintain healthy and safety of individuals.
- Transporting individuals into the community or when there is an emergency.
