“This job advertisement is for the purposes of facilitating an interview with the person to be supported. After a screening process, an interview will be determined by the person supported. The position being offered is a Community Habilitation specialist. The pay rate is $18-$19 an hour. The hours worked are afternoon/evening shift days can vary. This position will assist with the following:
Primarily assisting with daily living skills such as cooking, cleaning, and laundry. May occasionally involve scheduling appointments and providing transportation to them. Mileage is reimbursable. May include assisting the person in the community with activities and socialization, shopping skills.
Under the direction of the Person Supported and Fiscal Intermediary, this position is responsible for providing person directed services and supports as outlined in the person’s Self-Directed Budget, Life Plan, and Staff Action Plan(s).
Education:
- High School diploma, GED, or an acceptable TABE test score is preferred
