In a Community Support role you'll be joining the ranks of the many compassionate and caring individuals who provide daily support out in the community!

What would I be doing?

Assist Community Integration Specialist in providing Habilitation and Home Group (for individuals assigned) services to individuals in Day Habilitation.

• Provide direct service to members through direct outreach in homes (100% of your workday will be spent out in the community).
• Attends & transports the person we are supporting to all services based on individualized choice or other individualized Service Plan components (Doctor appointments, shopping, community activities, etc.).

Qualifications
-High school diploma or equivalent with six months experience preferable assisting individuals with disabilities, direct experience in a related field or one full year of college education in human services.

-Must successfully complete all required training including but not limited to First Aid/CPR, SCIP (Strategies for Crisis Intervention and Prevention) and Medication Administration., which we provide.

-Must be able to lift, move and transfer up to 50 pounds. Subject to background checks including but not limited to being fingerprinted/criminal history record check, NYS Child Abuse Registry, OPWDD Staff Exclusion List and Mental Health Law 16.34. Must meet LAI Vehicle Operator Requirements.

-Must have a personal vehicle & a Valid Driver's license of 2+ years of experience

Job Types: Full-time, Part-time

Pay: $17.25 - $18.75 per hour
