The Human Development Council is actively seeking a full-time Community Support Coordinatorfor our Frederick Day Program. We are seeking a caring, organized, creative individual. This is an administrative position, 40 hours a week Monday- Friday. A.A. Degree preferred - OR at least 2 years’ experience with individuals with intellectual disabilities. This position is full time and benefits.
Must be able to pass ACRE and CESP class within 1 year of employment.

Duties Include but are not limited to:
- Organize and over see Discovery/ Customization and/or Employment Services.
- Assist with staff daily scheduling.
- Work with Direct Care and/or Job Coach staff to develop and implement individuals goals.
- Plan and schedule all Community outings.
- Transport individuals to and from employment/ community events.
- Complete all reports on individuals within his/her assigned caseload.
- Review and approve Person-Centered Plan Review & Recommendations Reports, Person-Centered Plans, data sheets and evaluations.
- Represent the individuals and the agency at Person-Centered Plan Team meetings.
- Complete all assigned paperwork in an orderly and timely manner.
- Ensure that all DDA regulations are being followed within caseload.
- Accept extra duties when assigned.
- Provide room coverage and transportation coverage when needed.
- Work in coordination with other administrative staff to ensure that all records are properly maintained.
- Be available to meet with Instructors, Parents and Team members as requested.
- Ensure that all individuals rights are protected.
- Attend all Day Program Operational meetings.
- Attend Day/Residential/Transportation Coordinating meetings.
- Train direct care staff on specific Person-Centered in-services.
- Ensure implementation of Center Rules and Agency policy using disciplinary guidelines up to and including written reprimand when necessary.
- Good written and verbal communication skills
- Can work independently and as a team player
- Problem solving skills
- Take and pass all required in-services annually.
- Must be willing to get certified as a Behavioral Trainer (MANDT)
