Job description
The co-worker in this position will be responsible for identifying, educating and assisting Emergency department patients in St. Joseph's/Candler Hospital EDs to find, establish and maintain a Medical Home. The candidate will support the project leader in a variety of day-to-day operations including student support, data collection/reporting and project management.
3-6 month working in an acute hospital setting and/or 1 month. of supervised training as a Health C - Required
- Identifies health and social determinant barriers patients may have which causes them to utilize Emergency Departments as medical homes (i.e., Safety Net co-pays, no health insurance, education, chronic diseases, etc.
- Educates patient on the benefits of using a medical home for their primary care needs taking into consideration identified barriers, education levels and cultural norms
- Locates medical and non-medical services, agencies, organizations, etc. to meet identified patient needs
- Works one-on-one with each student health coach as a teacher and mentor. The Health Coach employee imparts knowledge about health care, Community Resources and motivational skills and facilitates successful outcomes with students in all aspects of health coach student responsibilities.
- Ensures that each follow-up is completed with each patient over a 6 month period. Follows the process flow for completing all Health Coach tasks.
