Position Summary
Orlando Health is seeking a Clinical Program Coordinator for their new Hospital Care at Home program with Orlando Regional Medical Center. As the Clinical Program Coordinator, you will be responsible for leading the patient acquisition efforts by identifying referral sources, patient selection, reviewing referrals, and coordinating admissions to the Hospital Care at Home unit.

Hospital Care at Home - Home RN
Hospital Care at Home admits patients requiring med-surg level care to their home, where they are cared for and monitored by nurses in the Patient Care Hub and in-home visits. This innovative care model utilizes Bluetooth technology to monitor patients remotely in the Patient Care Hub while providing in-home nursing care. Patients admitted to Hospital Care at Home are seen by a registered nurse in their home at least twice per day. Home nurses, in collaboration with nurses and physicians in the Patient Care Hub, provide hands-on assessment of patients, administer medications, and provide wound care or other treatments as ordered.

Responsibilities
Essential Functions
•Develops collaborative relationships with hospitals, physicians, team members and other referral sources.
•Develops and implements a communication plan that will lead to referrals and admissions to the Hospital Care at Home nursing unit.
•Analyzes referrals to determine if the patient meets admission criteria based on payor, chief complaint, resources needed and geographic catchment area.
•In coordination with the Patient Care Hub team supports the patient's admission from the hospital or emergency department to the Hospital Care at Home unit.
•Prioritizes potential admissions based on the patient's medical needs, payor source and geographic catchment area.
•Meets with patients/families to explain the Hospital Care at Home model and the admission process.
•Provides education to physicians, nursing team members, ancillary team members and other referral sources on the Hospital Care at Home model and acquisition process.
•Evaluates the referral and patient acquisition process to ensure the process is streamlined and efficient for all key team members. Recommends changes to processes as indicated.

•Serves as the point of contact for patient acquisition questions and provides guidance to team members.

Qualifications
Education/Training
Associate degree, Nursing Degree from an accredited college or university, or graduate of an approved LPN program
Licensure/Certification
Current licensure as a registered nurse in the State of Florida or Nurse Licensure Compact (NLC) or as a licensed practical nurse in the State of Florida.
Experience
One (1) year experience in the health care field.
