Hospital Care at Home – Home RN

Orlando Health is seeking a Clinical Program Coordinator for their new Hospital Care at Home program with Orlando Regional Medical Center. As the Clinical Program Coordinator, you will be responsible for leading the patient acquisition efforts by identifying referral sources, patient selection, reviewing referrals, and coordinating admissions to the Hospital Care at Home unit. The position will be based in St. Cloud hospital.
Hospital Care at Home – Home RN
Hospital Care at Home admits patients requiring med-surg level care to their home, where they are cared for and monitored by nurses in the Patient Care Hub and in-home visits. This innovative care model utilizes Bluetooth technology to monitor patients remotely in the Patient Care Hub while providing in-home nursing care. Patients admitted to Hospital Care at Home are seen by a registered nurse in their home at least twice per day. Home nurses, in collaboration with nurses and physicians in the Patient Care Hub, provide hands-on assessment of patients, administer medications, and provide wound care or other treatments as ordered.

Qualifications: Education/Training
Associate degree, Nursing Degree from an accredited college or university, or graduate of an approved LPN program

Licensure/Certification
Current licensure as a registered nurse in the State of Florida or Nurse Licensure Compact (NLC) or as a licensed practical nurse in the State of Florida.

Experience
