Job Summary:

The Hospital Liaison serves as Creative Hospice's contact for processing patient referrals from intake to admission.

The Hospital Liaison initiates positive customer relations, provides pertinent information via family contact to reflect the client's condition, and collects data from patients, physicians, discharge teams, social workers, and nursing homes so the clinical team can perform a comprehensive assessment for hospice eligibility.

The Hospital Liaison garners networking relationships with physicians, community health facilities as well as office-based practices, private practice, in group practices, in hospital settings, nursing homes, public housing representatives, assisted living facilities, and skilled care facilities within defined geographic territories.

Qualifications:
  1. Effective communication abilities including strong verbal and written group presentation skills.
  2. BA or BS degree preferred.
  3. Excellent sales skills with a proven track record of success over a multi-year period in healthcare sales preferred.
  4. At least three years of documented, successful sales to physicians, discharge planners, etc, or related experience in the healthcare industry, preferred.
  5. A minimum of two years experience in health care preferably, SNF, home care, or hospice is acceptable.
  6. A demonstrable history of building and influencing client relationships and goal achievement and knowledge of Medicare managed care, and insurance reimbursement is a plus.
  7. Employees must be computer literate, have a reliable car, valid driver's license, current car insurance, and a good driving record.
  8. A current Nursing License in the state of Georgia.
Responsibilities:
  1. Identifies and develops ongoing relationships with specific acute care hospitals and/or long-term care facilities to maintain liaison for the promotion of public awareness and understanding of the hospice program and philosophy.
  2. Assesses and evaluates patients for admission.
  3. Provides clinical contact with patients/families to promote options of care for end-of-life.
  4. Verifies patient's insurance coverage per agency policy.
  5. Collaborates with clinical staff in the acute care facility or long-term care facility to best manage pain and symptoms related to end-of-life diagnosis.
  6. Visits patients and families in the acute care facility or long-term care facility per hospital protocol.
  7. Serves as liaison between hospital and hospice team.
  8. Provides in-services to community agencies, physicians, and hospital referral services in support of hospice.
  9. Participates in agency quality improvement programs.
  10. Participates in regularly scheduled marketing meetings for coordination of services, exchange of information and problem solving, and to receive staff support and education as requested.
  11. Participates in hospice and home care programs providing staff orientation, training, and in-service.
  12. Applies the policies and procedures of the hospice and home care and the rules and regulations of Federal and State regulatory agencies and other certifying agencies in performing duties.
  13. Accepts other assignments deemed appropriate.
  14. Prepares reports for the Administrator as requested.
  15. Ensures that all care is provided with respect for patient rights.
  16. Reports all grievances and complaints made by patients or families to the appropriate person.
  17. Adheres to established personnel policies.
  18. Reports to all allegations of patient abuse and/or misappropriation of patient property.
  19. Follows standard precautions and infection control procedures.
  20. Reports all accidents and incidents observed.
  21. Identifies and responds appropriately to emergency situations.
  22. Observes the safety needs of the patients.
  23. Educates staff in the intake process, as needed.
  24. Attends in-services and meetings as required.
  25. Participates in in-service training as requested by the office.
  26. Communicates appropriate clinical information to the nursing staff and appropriate interdisciplinary team members regarding new admissions.
  27. Provides documentation of patient's visit on the clinical record in accordance with agency policy and state/federal regulations.
  28. In conjunction with the physician and the interdisciplinary team, develops an initial plan of care appropriate to the patient's disease.
  29. Addresses patient's care needs with families and significant others as appropriate.
  30. Collaborates with service providers furnishing contract services to the patient as needed.
  31. Participates in developing and updating policies and procedures as requested.
  32. Maintains confidentiality of necessary information.
  33. Thinks and acts calmly and logically to meet unusual occurrences of the job.
  34. Maintains proper documentation for billing.
  35. Interfaces with patients, families, and staff to ensure customer satisfaction.
  36. Communicates effectively with patients, families, and other health care providers.
  37. Maintains clinical competency by assuming responsibility for acquiring knowledge and experience to meet learning needs.
