Overview
The LPN Admissions Intake Coordinator supports the clinical home health and/or hospice teams to utilize a collaborative process to review, plan, monitor, and evaluate the options and services required to meet home health and/or hospice patients' health needs while coordinating available resources to promote quality and cost-effective outcomes through ongoing audits of medical records and clinical documentation to continually improve data quality in accordance with company standards and federal, state, and local standards, guidelines, and regulations.
Responsibilities
Operations Management and Administration
  • Completes all referrals by accessing the hospital medical records system (EMR) to gather H & P (history and physical), F2F (face-to-face), and relevant documentation then forwards to the Central Intake Department in the absence of hospital liaisons.
  • Collaborates with the marketing team to manage processes, protocols, and timeliness of all referrals to meet the comprehensive needs of the patients.
  • Works with schedulers and medical records team to coordinate all needs of staff and patients.
  • Interacts with patients and staff during phone calls, triages priorities, writes orders, and schedules visits as needed.
  • Monitors the home health and hospice EMR system and e-Fax and follows up on referrals and forwards appropriately to Central Intake or follows up as needed.
  • Assists with contacting new referrals and schedules new patients with priority on high risk and bundle patients. Notifies therapy when necessary.
  • Completes daily schedules on Saturday, Sunday, and Monday with a focus on maintaining appropriate staff members within the geographic boundaries.
  • Participates in on-call rotation as needed.
  • Completes customer service calls and monthly staff schedules.
  • Collaborates with hospital liaisons and discharge planners to assure timeliness of all referrals to meet the needs of the patients. Hospital liaison phones will be forwarded when they are off-duty.

Quality and Compliance Management
  • Audits and processes workflow/authorizations and maintains current and accurate medical records and clinical documentation to allow patients to be scheduled.
  • Reviews entire referral (i.e. H & P, nursing notes, etc.) and places coordination note in "POINT CARE" visit alert.
  • Serves as a resource to the clinical team for questions and concerns regarding patient care issues, federal and state regulations, and professional standards of practice in collaboration with the Administrator and/or Clinical Supervisors.
  • Informs the Administrator or designee immediately of any critical issues in accordance with notification policy.
  • Maintains knowledge of requirements of regulatory agencies, accrediting bodies, and third party payers.

People Management and Development
  • Ensures that there is a reasonable and sufficient plan for staffing should a staff member be absent.
  • Collaborates with the home health or hospice clinical team to operate within the allocated resources and budgeted goals, efficiencies, and productivity standards.
  • Fosters a positive work environment that attracts, retains, and motivates employees.
  • Monitors accountability with organization requirements prescribed by the Employee Handbook. Informs Administrator of employees who are routinely not observing these requirements in an effort to achieve compliance.
  • Responds timely and thoughtfully to questions, requests, and work product from departmental staff that has been provided for review and approval.

All other duties as assigned.
Qualifications
Education
  • Current unencumbered license for the state of Ohio to practice as a Licensed Practical Nurse (LPN) required.

Experience
  • One year experience in a home health, hospice, or long-term care healthcare setting required.
  • One year experience in a Medicare-certified home health agency required.
  • Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.
  • Proficiency with electronic medical records (EMR) preferred.

Other Requirements
  • Must be able to read, write, speak, and understand the English language.
  • Must possess a valid driver's license and acceptable driving record. Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.
