The Opportunity
HealthEZ is currently seeking a Care Case Manager, responsible for facilitating care to our members by assessing members needs, evaluating treatment plans and options, coordinating care and gauging progress. The Care Case Manager will work directly with our C-suite executive members and ensure streamlined care processes, improve clinical experiences, increase member satisfaction by providing healthcare service offerings to our C-suite executive members by effectively navigating the healthcare system and service offerings.
What you will do:
- Work with the Clinical Director, internal Medical Management team and vendors to ensure the achievement of high-quality care and outcomes for members
- Coordinate healthcare services that are personalized, timely, and effective for our members
- Handle every case with patience, understanding, and a desire to help our clients and members in achieving wellness and autonomy
- Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation
- Communicate patient needs to a variety of care team members and follow up accordingly
- Work collaboratively with members, families, physicians, and nurses to ensure high quality care is provided
- Advocate and raise awareness on behalf of clients and the needs of their members
- Drive change by identifying areas where performance improvement is needed
- Plan, develop and support organizational goals while upholding standards and current best practices
- Advocate for members by navigating the healthcare system assuring the best service is provided with appointments, physician orders, information related to treatment options and clinical status
- Evaluate medical necessity and appropriating resource availability
- Manage the cost of care effectively, provide timely and accurate information to members
- Facilitate care and treatment via scheduling, information sharing, and case coordination
- Manage case assignments, develop service plans, track progress, and execute case closures
- Track and record case information, complete forms, and produce reports
- Assess and act accordingly in response to treatment barriers and psychosocial issues
What we expect from you:
- Bachelor’s degree in nursing or healthcare-related discipline preferred.
- Active LPN or RN licensure is required
- 3-5 years of clinical experience, preferably in case management, or related experience
- Excellent clinical assessment and analytical skills
- Empathetic and supportive with mentorship and leadership skills
- Excellent interpersonal, verbal, and written communication skills
- Superb critical-thinking, organizational, and analytical skills
- Excellent time management skills; organized and able to prioritize
- Comfortable with building personal relationships and dedicated to helping others
Job Type: Full-time
Pay: $60,000.00 - $70,000.00 per year
