1st Choice Pediatric Home Care is seeking a hard working, empathetic, outgoing, and ambitious Pediatric Case Manager to advocate for our Pediatric Patients!
Responsibilities:
- Advocate for Pediatric Patient Care.
- Work with Physicians, Nursing staff, Hospitals, Therapist, and Insurance Companies to provide seamless care.
- Oversee physician orders to ensure being carried out correctly in the home.
- Meet with new families in order to coordinate start of care nursing services.
- Work with existing families in order to coordinate current nursing services.
- Submit prior authorization for insurance authorizations.
Qualifications:
- 1 year case management or similar experience preferred.
- Health Care experience preferred.
- Background in nursing, social work, family studies or similar field.
- Great communication skills.
- Great attention to detail.
- Ability to Multi-Task.
- Ability to handle stressful situations.
- Ability to work as a team in an open office space.

POSITION TYPES: Full Time M-F 7:45-3:45, also open to M-TH 7:45-3:45, option to work from home on Friday. 15 flexible use WFH days per calendar year.
COMPENSATION: $20.00 to $24.00
