SalaryAlpha Plus Home Health Services currently has open nursing positions across the entire County of San Diego including the South Bay areas, Eastlake, Otay Ranch, San Ysidro, Chula Vista, and more. We offer Per Diem positions allowing our clinical teams flexibility with their time, availability, and preferred region of coverage. We will cover cost of mileage whether you prefer to work in the region of choice or across all regions. We work with YOU!
Our experienced licensed nurses combine the science of nursing with the art of caring. They use the latest evidence based practices to inform, supervise, assess and provide skilled nursing services in the privacy of the patient's home.
Nurses are qualified and licensed to provide the following services (but not limited to):
● Wound care
● Diabetes Management
● Cardiovascular Disease Management
● I.V. Therapy
● Medication Management
● Injections
● Foley catheter care
● Ostomy Care and Instructions
● Glucose Monitoring
● Assessment, Teaching, Follow-up Support.
● Post Surgical Care
The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under
the supervision of a registered nurse. Responsibilities include following the plan of care,
providing treatments, and working collaboratively with all members of the team to achieve
optimal clinical outcomes for the patient and a positive experience for the patient and their
family.
ESSENTIAL JOB FUNCTIONS /RESPONSIBILITIES
1. Provides direct patient care as defined in the state Nurse Practice Act.
2. Implements a plan of care initiated by the registered nurse.
3. Provides accurate and timely electronic documentation using Home Health Care software
(WellSky) consistent with the patient’s assessment, plan of care following all regulatory and
Alpha Plus Home Health Services requirements.
4. Assesses and provides patient and family/caregiver education and information pertinent to
diagnosis and plan of care.
5. Participates in coordination of Home Health services, appropriately reporting the identified
needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse
and/or Clinical Supervisor/Nursing Supervisor.
6. Uses equipment and supplies effectively and efficiently.
7. Participates in personal and professional growth and development.
8. Performs other duties as assigned by the registered nurse.
● A graduate of an accredited school of professional nursing.
● Currently licensed to practice as an LVN in the state of CA.
● If IV certified, is currently certified in the state of CA.
● Has at least one (1) year acute nursing, medical surgical or equivalent experience with
current Home Health experience.
● Has a satisfactory reference from a previous (or current) employer, and/or professional
peers, school of nursing administrator/instructor.
● Must have excellent observation, good nursing judgment, and good oral and written
communication skills.
● Possesses basic computer skills and the ability to use a desktop or laptop computer, tablet
and or Smart Phone.
● Is self-directing and has the ability to work with little direct supervision.
● Is flexible and cooperative in fulfilling obligations of the role.
● Retains and maintains current CPR certification.
● Able to be discreet, demonstrate tact, patience, and good personal hygiene.
● Must be able to verbally communicate in the language of the client and document all client
related activity practices.
● Currently has or in the process of obtaining professional liability insurance.
● Is a current licensed driver with regular access to a reliable working and maintained motor
vehicle with current registration.
● Able to verify current automobile insurance in accordance with Alpha Plus Home Health
Services requirements.
● Retains and maintains good physical stamina and mental health.
● Able to present a pre-employment physician’s health clearance, including negative TB skin
tests and/or CXR and other tests as required by the statutes of the state of CA.
Pay: $45.00 - $50.00 per hour
Expected hours: No less than 4 per week
