JOB TITLE: Children's & Young Adult Care Coordinator

SERVICE AREA: Care Coordination Services

PROGRAM: Care Coordination

STATUS: Full Time (35 hours per week), Hybrid Position

FLSA DESIGNATION: Hourly

SALARY: $22.37-$23.43 per hour

REPORTS TO: Supervisor

DEGREE: Bachelor’s Degree in Social Work or related field.

EXPERIENCE: Minimum of 2 years relevant experience with SED/SPMI or chronic physical health conditions; experience with linkage and coordination of community services and emergency services; experience in substance abuse treatment valued

AGENCY REQUIREMENTS:

  • Valid NYS driver’s license
  • Use of personal vehicle during work hours


AGENCY EXPECTATIONS:

  • Demonstrates knowledge of the program, missions, and services of PCS.
  • Complies with all policies and procedures of PCS (Collective Bargaining Agreement, and corporate Compliance Policies) and all governmental and legal requirements.
  • Attends and participates in appropriate agency staff trainings and meetings.
  • Advocates for cultural diversity/sensitivity as appropriate.
  • Reports all suspected incidences of child abuse/neglect to NYS Mandated Reporter Hotline


GENERAL JOB RESPONSIBILITIES:

  • Provide direct social work contacts to 15 Non-Medicaid (NMD) youth ages 5-18 and 10-15 Health Home Medicaid Young Adults (YA) who are age 18-25.
  • Maintain consistent and on-going contact with youth, families, community providers, schools and any other services the youth is linked with while fulfilling unit of service expectations.
  • Coordinate on an ongoing basis with providers and conduct team meetings biannually to develop a plan of care that matches the YA/youth’s areas of interest and need and that will assist in removing barriers to progress
  • Assure youth has access to the Agency’s on call services for assistance after hours.
  • Provide on call services after hours as scheduled
  • Prepare professionally accurate and updated case documentation
  • Meet with Program Supervisor on a regular, planned basis
  • Participate in mandatory and optional trainings, including but not limited:
    • CANS-NY Certification (annual recertification)
    • NYS Mandated Reporter training (annually)
    • Netsmart CareManager Training
    • Engagement and Outreach
    • Safety in the Community
    • Trauma Informed Care
    • Person Centered Planning
    • Cultural Competency/Awareness
    • LGBTQ Issues
    • Meeting Facilitation
    • Monthly Group Supervisions
  • Advocate for cultural diversity, as appropriate
  • Perform other duties as assigned by Administration


ADDITIONAL RESPONSIBILITES BASED ON POPULATION:

NON-MEDICAID JOB RESPONSIBILITIES:

  • Conduct CANS-NY Assessments for the NMD youth to determine level of care and needs
  • Enter youth’s demographic and assessment information into CAIRS for state level tracking

HEALTH HOME MEDICAID JOB RESPONSIBILITIES:

  • Verify Medicaid status on a monthly basis or as frequently as needed
  • Provide developmentally appropriate support and linkages for employment, education, housing and other resources to help maintain youth’s health and develop skills for adulthood
  • Engage YA in their goal planning process
