Job description

The Patient Access Coordinator is responsible for assisting patients by navigating them through the health care system. Serves as a main point of contact for patients, referring physicians and caregivers. Facilitates patient appointments, including those made with labs, diagnostic areas, and specialty physicians. Patient Access Coordinators are committed to removing the patient’s barriers to care by identifying critical resources for patients, helping them navigate through health care services and systems, and promoting patient health. They work closely with the Care Team, which may include Providers, nurses, and other clinical staff to support positive patient health outcomes.


  • Establishes close relationships with and serves as primary point of contact for patients.
  • Assist patients in understanding their diagnosis, treatment options, and the resources available; including education eligible patients about appropriate clinical research studies and technologies.
  • Serves as an essential link between patients and all other care providers.
  • Facilitates appointment for consults and support services within established service standards, including Physician, Behavioral Health, and our Dieticians.
  • Develop concise patient summaries for use by the care team, and document recommendations made.
  • Responsible for outreach efforts to establish and maintain positive working relationships with key Care Team members (Providers, office staff, Care Coordinators, diagnostic staff, nurses, radiology staff, behavioral health staff, etc.) to facilitate patient care.
  • Communicates with Care Team members
  • Identify resources for clients to overcome barriers to care, such as transportation, housing, childcare arrangements, Home Health/Nursing Home arrangements.
  • Remain aware of current services offered by service providers, such as behavioral health, housing and employment assistance.
  • May meet with patients after primary care physician appointments to review and update care plan with Care Coordinator.
  • Meet with Care Team to discuss patient care issues and needs and facilitate patient health care.
  • Track patient information, schedules, files and forms in a confidential manner.
  • Track patients’ attendance at medical appointments, initiate outreach and missed appointment procedures, as necessary.
  • Being available for physicians in this setting as well as front desk coverage and relieve staff for lunches as needed.

  • Must have High School Diploma.
  • MA/LPN, preferred.
  • Excellent typing, computer skills, and data entry knowledge, preferred.
  • Excellent organizational skills required.
