How you can make a bigger difference:
The Kaplan House (Manhattan) and Cedar Knolls House (Bronx) provides therapeutic and support services to adolescent's ages 16-21 who are referred through the Administration for Children's Services. Our residents have experienced significant trauma and often struggle with managing their emotional and behavioral needs. The program provides an array of services including medical, clinical, education/vocation, recreation, substance use and additional support services that emphasize family interaction and strengthening of community support networks, as well as increasing independent living skills.
The Caseworker will provide aftercare support and services for discharged youth and their families upon youth achieving their permanency goal. The Caseworker is responsible for conducting monthly home visits/face to face contacts on at least a bi-weekly basis starting from when the youth is placed on Trial Discharge. The Caseworker will then continue to monitor youth for a 6-month period post Final Discharge. Youth may be residing in Supportive Housing, NYCHA Housing, and Section 8 apartments located throughout the five boroughs. Some responsibilities include, but are not limited to:
  • Work 1:1 with youth to develop individualized goals based on their interests and strengths and take measurable steps towards their goals.
  • Connect youth to quality schools, employers, programs, resources, and opportunities in connection with their goals.
  • Provide advocacy and outreach with community based providers and complete any necessary referrals.
  • Collaborate with the treatment team throughout the discharge planning process.
  • Provide connection to resources specific to clients' needs.
  • Attend Family Team Conferences, Discharge Conferences, and any other pertinent meetings.
  • Re-engage disconnected youth and assist them with their desired goals.
  • Work closely with the program's College & Career Development Specialist and Youth Empowerment Coaches to expose youth to post-secondary pathways, such college and/or accredited vocational programs.
  • Support youth with their transition to adult housing included but not limited to: moving in, obtaining furniture, and supporting them with maintaining housing through IL skills (budgeting, opening bank accounts, etc.)
  • Other duties as assigned

Qualifications:
The Jewish Board is currently looking for talented individuals of all cultures, religions, races, and gender expressions with the following qualifications:
  • Bachelor's degree required
  • Experience working with at-risk or system-involved young people is strongly preferred.
  • A current, valid and clean Driver's license is required for this position
